PROVIDE BASIC NEEDS ASSISTANCE, CASE MANAGEMENT SUPPORT AND HOUSING OPTIONS TO UNACCOMPANIED HOMELESS YOUTH SO THEY COMPLETE THEIR HIGH SCHOOL EDUCATION AND/OR INCREASE THEIR SELF-SUFFICIENCY.

PROJECT 1649 PROVIDES BASIC NEEDS ASSISTANCE FOR MORE THAN 300 UNACCOMPANIED HOMELESS YOUTH IN ROCK COUNTY WI EACH YEAR AND PROVIDES DIRECT ASSISTANCE AND CASE MANAGEMENT TO APPROXIMATELY 90 YOUTH. THE ORGANIZATION ALSO OPERATES TWO SUPERVISED TRANSITIONAL LIVING HOMES (ONE FOR FEMALES AND ONE FOR MALES) WHICH SERVE 35-40 YOUTH ANNUALLY. PROJECT 1649 PROVIDES YOUTH (TRANSITIONAL LIVING HOME RESIDENTS, COMMUNITY CASE MANAGEMENT PARTICIPANTS, AND IN-SCHOOL WORKSHOP PARTICIPANTS) WITH OPPORTUNITIES TO LEARN AND DEVELOP IMPORTANT INDEPENDENT LIVING/LIFE SKILLS. IN 2021, THE TRANSITIONAL LIVING HOMES SERVED 33 YOUTH. 88% OF RESIDENTS GRADUATED OR WERE ATTENDING SCHOOL REGULARLY, THE PROGRAM ACHIEVED A 90% POSITIVE EXIT RATE, 100% OBTAINED/INCREASED EMPLOYMENT AND 95% OF YOUTH INCREASED THEIR SELF-SUFFICIENCY SCORE IN TWO OR MORE OF FIVE KEY AREAS.
