TO PROVIDE EDUCATION, INFORMATION, SUPPORT SERVICES AND PROGRAMS TO HEL IMPROVE QUALITY OF LIFE FOR PEOPLE WITH PARKINSON'S AND THEIR FAMILIES, LOVED ONES AND SUPPORTERS AND TO ENHANCE PUBLIC UNDERSTANDING OF THE CHALLENGES FACED BY THOSE AFFECTED.

Part 3 - Line 4a

TO PROVIDE HOME CARE LIVING ASSISTANCE TO LOCAL PARKINSON PATIENTS AND THEIR CARE-GIVER WHEN THE NEED ARISES. HOME CARE CAN BE USED WHEN CARE-GIVER HAS TO BE AWAY FOR A LONG PERIOD OF TIME OR IF THE PARKINSON PATIENT IS ALONE AND NEEDS HELP WITH HOME CARE.


ROCK STEADY BOXING IS A NON CONTACT BOXING PROGRAM THAT PROVIDES PARKINSON PATIENTS WITH A WEEKLY EXERCISE PROGRAM AT A LOCAL GYM FACILITY. THIS PROGRAM IS SPECIFICALLY FOR PARKINSON PATIENTS AND IT CAN DRAMATICALLY SLOW DOWN SYMPTOMS AND IMPROVE PHYSICAL HEALTH AGILITY AND DAILY FUNCTIONING FOR A PARKINSON PATIENT. THE COST OF THIS PROGRAM IS $89 PER MONTH FOR EACH MEMBER. MEMBER PAYS $20 AND LSPS PAYS $69 FOR EACH MEMBER.


TO PROVIDE A WEEKLY EXERCISE PROGRAM FOR PARKINSON PATIENTS. THIS PROGRAM IS A FLOOR EXERCISE PROGRAM SPECIFICALLY FOR PARKINSON PATIENTS AND IS LOCATED AT THE SENIOR CITIZEN'S CENTER. MONTHLY COST PER MEMBER IS $40 AND LSPS PAYS ALL MEMBER'S MONTHLY FEES.
