Describe the Organization's Program Activity:
AVERA AT HOME PROVIDES A VARIETY OF HOME CARE SERVICES THAT MAKE IT POSSIBLE FOR INDIVIDUALS TO REMAIN IN THEIR HOME RATHER THAN LONG-TERM RESIDENTIAL OR NURSING HOME FACILITIES. AVERA AT HOME IS A FULL-SERVICE, ACCREDITED HOME MEDICAL EQUIPMENT PROVIDER OFFERING A VARIETY OF MEDICAL AND HEALTH-RELATED ITEMS FOR PURCHASE OR RENT. ADDITIONALLY, IT PROVIDES QUALITY HOME HEALTH CARE WITH A FULL RANGE OF SKILLED NURSING, THERAPY, AND HOME HEALTH AIDE SERVICES.FINALLY, AVERA AT HOME PROVIDES HOSPICE CARE FOR PEOPLE FACING A LIFE-LIMITING ILLNESS OR INJURY. QUALITY, COMPASSIONATE CARE IS PROVIDED IN THE PATIENT'S HOME OR WITHIN A 16-BED RESIDENTIAL AND INPATIENT CARE HOSPICE FACILITY IN SIOUX FALLS, SOUTH DAKOTA.AVERA AT HOME SERVICES WERE INVALUABLE DURING THE COVID PANDEMIC. THROUGHOUT THE PANDEMIC, MORE THAN 6,000 COVID-19 PATIENTS RECEIVED CARE AT HOME VIA VIRTUAL METHODS, INCLUDING PHONE, TEXT AND VIDEO CHAT INTERACTIONS. AT THE PEAK OF COVID, UP TO 1,400 PATIENTS WERE BEING MONITORED AT HOME AT ANY ONE TIME. MANY OF THE PATIENTS WOULD HAVE BEEN HOSPITALIZED OTHERWISE. THE AVERA AT HOME HOME MONITORING HELPED KEEP PEOPLE OUT OF THE HOSPITAL WHILE STILL GETTING THE HEALTHCARE THEY NEED. AVERA AT HOME TEAMS HELPED IN MANY WAYS:- PROVIDING FOLLOW-UP CARE FOR PATIENTS AFTER THEY LEFT HOSPITALS, CLINICS AND EMERGENCY ROOMS- SETTING PATIENTS UP WITH IMPORTANT MEDICAL EQUIPMENT, INCLUDING OXYGEN- SUPPORTING PATIENTS WITH ONGOING, OFTEN DAILY, VIRTUAL VISITS OR PHONE CALLS- VIRTUAL MONITORING OF VITAL SIGNS FOR AT-HOME PATIENTS.THE ONGOING CONNECTION, EVEN WHEN VIRTUAL, GAVE PATIENTS THE CONFIDENCE TO CONTINUE THEIR RECOVERY AT HOME.
