ALTERNATIVES PROMOTES THE INDEPENDENCE AND QUALITY OF LIFE FOR OLDER ADULTS, ADULTS WITH DISABILITIES, AND THEIR FAMILIES.

Part 3 - Line 4a

CARE COORDINATION PROGRAM - PROVIDES ONGOING ASSESSMENT AND SERVICE COORDINATION FOR OLDER ADULTS TO REMAIN INDEPENDENT. IN FY22, ALTERNATIVES RECEIVED 14,260 REFERRALS FOR SUPPORT. ALTERNATIVES INCREASED ONGOING SUPPORT TO A CASELOAD OF MORE THAN 2,800 PARTICIPANTS. AS A PROGRAM, CARE COORDINATION ASSISTED 86% OF SENIORS ELIGIBLE FOR NURSING HOME CARE TO REMAIN IN THEIR HOME. 95% OF PARTICIPANTS AND CAREGIVERS INDICATE THEY WOULD RECOMMEND ALTERNATIVES' SERVICES TO OTHERS.


ADULT PROTECTIVE SERVICES - CONDUCTS INTAKE, INVESTIGATION, CASEWORK AND INTERVENTION TO PREVENT FURTHER ABUSE, NEGLECT OR FINANCIAL EXPLOITATION OF OLDER ADULTS OR OF ADULTS WITH DISABILITIES (AGE 18-59). IN FY22, ADULT PROTECTIVE SERVICES INVESTIGATED 984 CASES OF WHICH 28% WERE SUBSTANTIATED FOR ABUSE, NEGLECT OR EXPLOITATION.


OMBUDSMAN - PROVIDES ADVOCACY, CONSULTATION AND FOLLOW UP FOR MORE THAN 11,000 NURSING HOME RESIDENTS IN A SIXTEEN COUNTY SERVICE AREA. OMBUDSMEN PROVIDED DIRECT ASSISTANCE TO MORE THAN 1,900 NURSING HOME RESIDENTS TO RESOLVE COMPLAINTS ABOUT THEIR CARE. OMBUDSMEN PROVIDED A REGULAR PRESENCE IN EACH FACILITY TO ADDRESS SYSTEMIC CHALLENGES TO CARE OF NURSING HOME RESIDENTS.
