HAVEN HOMES, INC. WAS ORGANIZED FOR THE PURPOSE OF PROVIDING NURSING HOME CARE FOR THE ELDERLY. IT IS LOCATED IN MAPLE PLAIN, MINNESOTA AND HAS 64 BEDS IN SERVICE. DURING 2021, TOTAL RESIDENT DAYS WERE 17,133 (73.3% OCCUPANCY). UTILIZATION BY PAYOR SOURCE WAS AS FOLLOWS: 48% MEDICAL ASSISTANCE, 7% MEDICARE, 33% PRIVATE PAY AND 12% HMO.
