CHOICE HOME CARE, INC. IS A MINNESOTA CLASS A LICENSED AND MEDICARE CERTIFIED HOME HEALTH CARE AGENCY PROVIDING SERVICES IN EIGHT COUNTIES IN SOUTHEASTERN MINNESOTA. OUR NURSING SERVICES RANGE FROM MEDICATION MANAGEMENT TO POST HOSPITAL AND POST NURSING HOME CARE, WOUND CARE, AND DISEASE MANAGEMENT. CHOICE HOME CARE STAFF ARE COMMITTED TO MAINTAINING THE DIGNITY, COMFORT, SAFETY, AND INDEPENDENCE OF EACH CLIENT BY PROVIDING THE HIGHEST QUALITY PROFESSIONAL HOME HEALTH SERVICES. WE HELP FAMILY MEMBERS UNDERSTAND THE CHANGED CONDITION AND ILLNESS OF THEIR LOVED ONES THEREBY REDUCING THEIR STRESS WHILE DEALING WITH THE NEEDS OF THEIR LOVED ONES. WE PRIDE OURSELVES IN BEING RESPONSIVE TO THE HOME HEALTH SERVICE NEEDS OF THE COMMUNITIES IN WHICH WE DO BUSINESS. CHOICE HOME CARE STOPPED PROVIDING HOME CARE SERVICES TO CLIENTS IN FEBRUARY 2015.
