MINNESOTA NETWORK OF HOSPICE & PALLIATIVE CARE PROMOTES ADVANCE CARE PLANNING, PALLIATIVE CARE AND HOSPICE THROUGH EDUCATION TO INCREASE AWARENESS AND ACCESSIBILITY TO THESE SERVICES FOR EVERYONE.

"PROGRAM" - EDUCATION FOR HOSPICE & PALLIATIVE CARE PROFESSIONALS MNHPC PROVIDES EDUCATION, TOOLS, AND RESOURCES TO HOSPICE AND PALLIATIVE CARE PROVIDERS TO ENSURE THAT PATIENTS AND THEIR FAMILIES HAVE ACCESS TO HIGH QUALITY SERIOUS ILLNESS AND END-OF-LIFE CARE. IN 2021, MNHPC TRAINED OVER 1,000 INDIVIDUALS THROUGH VIRTUAL CONFERENCES, SUMMITS, AND WEBINARS. MNHPC ALSO CO-CONVENED TWO MINNESOTA SERIOUS ILLNESS ACTION NETWORK FORUMS TO ADDRESS ISSUES RELATED TO PALLIATIVE CARE PAYMENT AND POLICY AND CAREGIVING. OVER 230 INDIVIDUALS PARTICIPATED IN THE FORUMS. "PROGRAM" - EDUCATION AND TRAINING FOR PATIENTS, CAREGIVERS, AND CONSUMERS MNHPC HOSTED A VIRTUAL SCREENING OF THE FILM, "CAREGIVER A LOVE STORY," FOLLOWED BY PANEL DISCUSSIONS WITH CAREGIVERS AND HOSPICE PROVIDERS. 302 INDIVIDUALS ATTENDED THIS EVENT AND RECEIVED INFORMATION AND EDUCATION ON HOW HOSPICE SUPPORTS CAREGIVERS. MNHPC ALSO GAVE THREE PRESENTATIONS TO COMMUNITY-BASED ORGANIZATIONS. "PROGRAM" - RESOURCE DEVELOPMENT MNHPC MAINTAINS A WEBSITE THAT IS DESIGNED TO PROVIDE INFORMATION TO MNHPC MEMBERS, WHO ARE HOSPICE AND PALLIATIVE CARE PROVIDERS, AS WELL AS CONSUMER BASED INFORMATION FOR PATIENTS, FAMILIES, AND CAREGIVERS. RESOURCES FOR HEALTH CARE PROFESSIONALS INCLUDE INFORMATION ABOUT REGULATIONS, POLICIES, AND BEST PRACTICES. INFORMATION FOR CONSUMERS INCLUDES A HOSPICE CONSUMER GUIDE, FREQUENTLY ASKED QUESTIONS, INFORMATIONAL VIDEOS DESIGNED TO BE CULTURALLY AND LINGUISTICALLY APPROPRIATE, HOSPICE AND PALLIATIVE CARE DIRECTORIES, AND INFORMATION ABOUT PEDIATRIC PALLIATIVE CARE.
