SEE SCHEDULE OMAYO CLINIC HEALTH SYSTEM-FRANCISCAN HEALTHCARE FOUNDATION, INC. IS A PART OF THE MAYO CLINIC HEALTH SYSTEM. MAYO CLINIC HEALTH SYSTEM IS A FAMILY OF CLINICS, HOSPITALS AND HEALTH CARE FACILITIES SERVING COMMUNITIES IN MINNESOTA, IOWA, WISCONSIN AND GEORGIA. AS PART OF MAYO CLINIC, THE HEALTH SYSTEM OFFERS CARE DESIGNED FOR THE WHOLE BODY CLOSE TO THE PATIENT'S HOME, WHEN THEY NEED IT. A MULTIDISCIPLINARY TEAM OF EXPERTS HAS READY ACCESS TO KNOWLEDGE, RESOURCES, AND EXPERTISE OF MAYO CLINIC AND COLLABORATES ON PROVIDING EXACTLY THE CARE THE PATIENT NEEDS. MAYO CLINIC HEALTH SYSTEM SITES EMPLOY APPROXIMATELY 1,000 PHYSICIANS AND 15,000 ALLIED HEALTH STAFF. IN ADDITION, MAYO CLINIC PHYSICIANS FROM MULTIPLE SPECIALTIES REGULARLY SEE PATIENTS AT HEALTH SYSTEM FACILITIES. BESIDES HAVING HEALTH CARE FACILITIES IN MORE THAN 60 COMMUNITIES, MAYO CLINIC HEALTH SYSTEM INCLUDES 19 OWNED HOSPITALS, TWO OWNED NURSING HOMES, PHYSICIAN SERVICE AGREEMENTS WITH ONE ADDITIONAL HOSPITAL, AND CONTRACT MANAGEMENT AGREEMENTS WITH TWO ADDITIONAL HOSPITALS AND ONE NURSING HOME. SINCE ITS INCEPTION IN 1992, MAYO CLINIC HEALTH SYSTEM HAS GROWN INTO ONE OF THE MOST SUCCESSFUL REGIONAL HEALTH CARE SYSTEMS IN THE UNITED STATES. MORE THAN 600,000 UNIQUE PATIENTS ACCOUNT FOR MORE THAN 2.6 MILLION VISITS ANNUALLY IN MAYO CLINIC HEALTH SYSTEM. IT IS ALSO A NATIONAL LEADER IN QUALITY AND SAFETY EFFORTS.THE FOUNDATION IS OPERATED EXCLUSIVELY FOR THE BENEFIT OF, TO PERFORM THE FUNCTIONS OF, OR TO CARRY OUT THE PURPOSES OF MAYO CLINIC HEALTH SYSTEM-FRANCISCAN MEDICAL CENTER, INC. (FMCI), AND ANY TAX EXEMPT AFFILIATED OR SUBSIDIARY ENTITY OF FMCI; TO SOLICIT AND RECEIVE GIFTS, GRANTS AND CONTRIBUTIONS FOR AND ON BEHALF OF FMCI, AND ANY TAX EXEMPT AFFILIATED OR SUBSIDIARY ENTITY OF FMCI; AND TO SUPPORT THEIR CHARITABLE PROGRAMS AND RELATED ACTIVITIES THROUGH THE EXPENDITURE OF FUNDS RECEIVED AND INCOME THEREON OR DISTRIBUTION OF OTHER PROPERTY RECEIVED. THE CORPORATIONS' EXPENDITURES SHALL BE FOR (A) THE PURCHASE, LEASE RENOVATION, OR MODERNIZATION OF BUILDINGS, FURNITURE, FIXTURES, AND EQUIPMENT, (B) EDUCATIONAL PROGRAMS, AND (C) IN ALL OTHER WAYS AIDING AND SUPPORTING TAX EXEMPT HEALTH ORGANIZATIONS AND THEIR RELATED ACTIVITIES IN THE COUNTY AND SURROUNDING COUNTIES OF LA CROSSE, WISCONSIN INCLUDING THOSE LOCATED IN MINNESOTA AND IOWA. IN THE PAST YEAR GRANTS WERE AWARDED TO SUPPORT NUMEROUS FMCI PROGRAMS AND SERVICES INCLUDING DIABETES, CANCER, NEWBORN CENTER, INTERNAL MEDICINE, BEHAVIORAL HEALTH, CARDIOLOGY, FAMILY PRACTICE, CARDIAC CARE, FAMILY BIRTHPLACE, INTENSIVE CARE, WOMEN'S HEALTH, ELDER CARE AND EMERGENCY CARE. GRANTS WERE ALSO PROVIDED TO GERARD HALL, HOSPICE, ST. CLARE HEALTH MISSION, SPIRITUAL CARE AND OTHER PROGRAMS; PATIENTS AND FAMILIES THROUGH THE THUNDER RIDE, OASIS, ST. FRANCIS AND DANNY MITBY FUNDS; STAFF EDUCATION AND MEDICAL RESEARCH; FACILITIES AND EQUIPMENT; AND SUPPORT OF COMMUNITY INITIATIVES INCLUDING FINANCIAL ASSISTANCE TO NON-PROFIT ORGANIZATIONS FOR THE PURCHASE OF AUTOMATIC EXTERNAL DEFIBRILLATORS. ALTHOUGH WE BELIEVE FUNDRAISING TO BE ONE OF OUR PROGRAM SERVICES, THE COSTS DIRECTLY ASSOCIATED WITH FUNDRAISING EFFORTS ARE REPORTED SEPARATELY IN AN EFFORT TO PROVIDE PROSPECTIVE DONORS WITH THE INFORMATION NECESSARY TO MAKE INFORMED PHILANTHROPIC DECISIONS.
