WE ALSO PROVIDED DISABILITY AWARENESS TRAINING FOR THE PUBLIC TO IMPROVE INTERACTION BETWEEN THE PUBLIC AND PEOPLE WITH DISABILITIES. TOTAL SERVED 3694


NURSING FACILITY TRANSITION PROGRAM - THIS PROGRAM PROVIDES INDIVIDUALS, REGARDLESS OF THEIR DISABILITY, AN OPPORTUNITY TO CHOOSE COMMUNITY-BASED LIVING AS A VIABLE ALTERNATIVE TO NURSING HOME RESIDENCY. TOTAL SERVED 0
