OUR EXEMPT PURPOSE IS TO PROVIDE MEDICAL SERVICES TO OUR CLIENTS. THE NUMBER OF MEDICAL ENCOUNTERS FOR THE YEAR WAS 1,160.


OUR EXEMPT PURPOSE IS TO PROVIDE BEHAVIOR HEALTH SERVICES TO OUR CLIENTS. THERE WERE 813 BEHAVIOR HEALTH ENCOUNTERS FOR THE YEAR. IN ADDITION THERE WERE A TOTAL OF 348 SUICIDE RISK ASSESMENTS COMPLETED. GRANTS GARRETT LEE SMITH, ZERO SUICIDE, NATIVE CONNECTIONS, AND 4IN1 MH & SA.


INCLUDES ALL OTHER SERVICES PROVIDED TO PATIENTS.
