TO HELP CENTRAL OHIOANS WITH DIABETES TO DETECT THEIR CONDITION, PREVENT ONSET AND COMPLICATIONS, AND LEARN TO LIVE WELL WITH THE CHALLENGE OF DIABETES. ACTIVITIES ARE DIABETES EDUCATION, COOKING CLASSES, DIABETES SCREENINGS AND CAMPS FOR CHILDREN WITH DIABETES.

HEALTH EDUCATION: NEARLY 2000 INDIVIDUALS ATTACHED DIABETES-SELF MGT EDUCATION & SUPPORT CLASSES, NUTRITION FOR DIABETES, COOKING DEMONSTRATIONS, BLOOD GLUCOSE TRAINING, HEALTH EDUCATION WORKSHOPS, DIABETES FUNDAMENTAL CLASSES, YMCA DIABETES PROGRAM, INCH BY INCH PREVENTION PROGRAM AND NUTRITION COUNSELING SESSIONS. 1000 INDIVIDUALS RECEIVED CASE MGT SERVICES, MENTAL HEALTH COUNSELING, PARTICIPATED IN EMPOWERMENT GROUPS AND RECEIVED INFORMATION AND REFERRALS; OR PARTICIPATED IN INDIVIDUAL CONSULTATION AND COUNSELING. 83% OF THOSE IN OUR CLASSES REPORT THAT THEIR NUTRITION MGT AND QUALITY OF LIFE HAS IMPROVED & 100% OF THE RESPONDENTS REPORTED IMPROVED A1C BLOOD GLUCOSE LEVELS. OVER 70% OF OUR CLIENTS ARE UNINSURED OR UNDERINSURED, MEETING OUR MISSION OF ASSISTING THE UNDERSERVED TO LEARN TO LIVE WELL WITH THE CHALLENGE OF DIABETES. OUR DIABETES SELF-MANAGEMENT EDUCATION PROGRAM IS RECOGNIZED BY THE AMERICAN DIABETES ASSOCIATION.


CAMP: 165 CHILDREN ATTENDED KIDS CAMP, STEPPING STONES OR CAMP HAMWI, 55% OF WHOM RECEIVED FULL OR PARTIAL FINANCIAL AID. 90% OF THE CHILDREN ATTENDING CAMP PROGRAMS INCREASED AND ADHERED TO SELF-MANAGEMENT SKILLS LEARNED AT CAMP. ALL CAMPS ARE ACCREDITD BY THE AMERICAN CAMPING ASSOCIATION. THE CAMP OFFERS DIABETES EDUCATION BASED ON A LIFE-STYLE MANAGEMENT MODEL THAT CAN BE BILLED TO INSURANCE COMPANIES. EVERY CAMPER IS SUBSIDIZED BY THE AGENCY. YOUTH: COUNSELING AND CASE MANAGEMENT SERVICES FOR YOUTH AND FAMILIES; HOLIDAY PARTIES FOR CHILDREN WITH DIABETES WITH PARENT AND YOUTH EDUCATION PROVIDED. OUR WOMEN'S BOARD PROVIDES COLLEGE SCHOLARSHIP MONEY TO YOUTH WITH IDABETES WHO SHOW COMPETENT DIABETES MANAGEMENT AND DEMONSTRATE EXEMPLARY SCHOLASTIC ACHIEVEMENT.


DETECTION: THERE WERE OVER 565 SCREENING EVENTS IN 2016, 4652 INDIVIDUALS LEARNED THEIR DIABETES STATUS AND UNDERSTOOD THE WARNING SIGNS. 1900 OF THOSE SCREENED RECEIVED FOLLOW-UPS AND LINKAGE BECAUSE THEY SCREENED HIGH. WE TRAINED 318 VOLUNTEERS AND PHARMACY STUDENTS TO ASSIST WITH PROVIDING DETECTION SCREENING. 100% OF EVERYONE WHO SCREENED ABNORMALLY RECEIVED FOLLOW-UP SERVICES. WE OFFER COMMUNITY SCREENING ESPECIALLY TARGETED TO HIGH RISK POPULATIONS TO HELP PEOPLE DETECT UNDIAGNOSED AND UNCONTROLLED DIABETES.
