HOME CARE SERVICES - TO PROVIDE IN-HOME PERSONAL CARE, HOME MANAGEMENT, HOUSE CLEANING, AND STRENGTHENING THE FUNCTIONING OF INDIVIDUALS.


TRANSPORTATION - PROVIDING TRANSPORTATION FOR MEDICAL PERSONNEL, AND SENIOR FRIENDSHIP AND SOCIAL CENTER ACTIVITIES.


CASE MANAGEMENT - PROVIDE SOCIAL WORKER SERVICES TO ASSESS SENIORS NEEDS AND HELP THEM TO FIND THE SERVICES NEEDED TO KEEP THEM HEALTHY AND TO REMAIN INDEPENDENT IN THEIR HOMES.


NUTRITION - MEALS ON WHEELS PROGRAM DELIVERING HOT MEALS FIVE DAYS A WEEK TO THE AGED AND HOME BOUND INCLUDES CONGREGATE MEALS PROGRAMS PROVIDING ACTIVITIES AND TRIPS. ADULT DAY CARE - PROVIDES A SAFE PLACE FOR SENIORS TO GO AND REMAIN ACTIVE WHILE GIVING THEIR CAREGIVERS RESPITE. LIFELONG LEARNING - PROVIDES LEARNING ACTIVITIES FOR MORE ACTIVE SENIORS AND IS THE GATEWAY TO OTHER SERVICES. VOLUNTEER SERVICES - COORDINATES VOLUNTEERS TO HELP SERVE ALL OF THE SENIORS NEEDS. HOME REPAIR - PROVIDES MINOR HOME REPAIRS AND MODIFICATIONS TO HELP THE SENIORS REMAIN IN A SAFE LIVING CONDITION.
