OUR MISSION IS TO PROVIDE LIFE-LONG SOLUTIONS FOR THE SOCIAL, EDUCATIONAL, VOCATIONAL, AND RESIDENTIAL NEEDS OF ADULTS WITH INTELLECTUAL AND DEVELOMENTAL DISABILITIES.

PROVIDER SERVICES THE SOARING EAGLE CENTER IS A MEDICAID WAIVER PROGRAM PROVIDER OFFERING SERVICES FOR HCS HOME AND COMMUNITY-BASED SERVICES AND TEXAS HOME LIVING. OUR SUPPORT INCLUDES COMMUNITY ASSISTANCE, RESPITE CARE, TRANSPORTATION, NURSING, SPEECH AND OCCUPATIONAL THERAPIES, AND BEHAVIORAL SUPPORT. THROUGH A CONTRACT WITH THE TEXAS HEALTH AND HUMAN SERVICES COMMISSION, WE SUPERVISE AND COMPENSATE THE CAREGIVERS AND THERAPISTS DELIVERING THESE SERVICES TO INDIVIDUALS ENROLLED IN THESE WAIVER PROGRAMS. IN 2023, THE SOARING EAGLE CENTER SERVED A TOTAL OF 34 CLIENTS.


ASSOCIATES PROGRAMMING INDIVIDUALIZED SKILLS AND SOCIALIZATION ISS LICENSED LOCATION AND DAY PROGRAM THE ISS AND DAY PROGRAM MEETS MONDAY THROUGH FRIDAY FROM 900 A.M. TO 300 P.M. AND IS OPEN TO ADULTS WHO HAVE COMPLETED HIGH SCHOOL AND HAVE BEEN DIAGNOSED WITH AN INTELLECTUAL AND DEVELOPMENTAL DISABILITY IDD. AFTER HIGH SCHOOL, THE STATE DOES NOT PROVIDE PROGRAMS SPECIFICALLY FOR ADULTS WITH IDD, AND OUR PROGRAM FILLS THIS CRUCIAL GAP. PARTICIPANTS IN THE PROGRAM ATTEND CLASSES TO ENHANCE THEIR ACADEMIC SKILLS, TAKE PART IN FIELD TRIPS AND COMMUNITY ACTIVITIES, WORK ON IMPROVING DAILY LIVING SKILLS, AND BUILD LIFELONG FRIENDSHIPS THROUGH SHARED EXPERIENCES, PLAY, AND LAUGHTER. IN 2023, THE ISS AND DAY PROGRAM SERVED A TOTAL OF 36 ADULTS.
