NORTH ALABAMA COMMUNITY CARE, INC.'S (NACC) MISSION IS TO IMPROVE HEALTH, WELFARE, AND EFFICIENCY OF CARE PROVIDED TO NORTH ALABAMA ELIGIBLE MEDICAID RESIDENTS. NACC UTILIZES THE HEALTHCARE SYSTEM AND COMMUNITY RESOURCES TO PROVIDE PATIENT CENTERED CARE PLANS TO HIGH RISK MEDICAID PATIENTS. NACC STRIVES TO REDUCE INAPPROPRIATE UTILIZATION AND COSTS ASSOCIATED WITH EMERGENCY ROOM AND HOSPITAL VISITS BY ESTABLISHING A GOAL ORIENTED SELF- MANAGEMENT SYSTEM WITH EACH PATIENT.
