THE ORGANIZATION IS A SUPPORT SERVICE BROKERAGE WHICH ASSISTS ADULTS WITH INTELLECTUAL AND/OR DEVELOPMENTAL DISABILITIES TO LIVE AS INDEPENDENTLY AS POSSIBLE IN THE COMMUNITY. SERVICES INCLUDE PLANNING, OBTAINING AND MONITORING OF INDIVIDUAL SERVICES RELATED TO EACH INDIVIDUAL'S DISABILITY. PERSON-CENTERED PLANNING IS USED TO DETERMINE NEEDS AND RESOURCES. INDIVIDUALS SERVED BY THE SUPPORT SERVICE BROKERAGE MUST BE DETERMINED ELIGIBLE AND REFERRED FROM THE COUNTY DEVELOPMENTAL DISABILITY PROGRAM IN THE COUNTY WHERE THEY LIVE.
