MENTAL HEALTH COLLABORATIVE - PROVIDES MENTAL HEALTH THROUGH A COOPERATIVE WITH BILINGUAL INTERNATIONAL ASSISTANT SERVICES; SAINT FRANCIS COMMUNITY SERVICES; SAINT LOUIS UNIVERSITY MEDICAL FAMILY THERAPY PROGRAM; AND ST. LOUIS PSYCHOANALYTIC INSTITUTE. THE COOPERATIVE OPERATES FROM A NEWLY RENOVATED BUILDING AT 1015 S. COMPTON, THE BUILDING ADJACENT TO CASA'S CHOUTEAU AVENUE SITE.
THE CLINIC IS THE ENTRY POINT FOR PATIENTS COMING TO CASA FOR CARE. PATIENTS RECEIVE PRIMARY AND ACUTE CARE FROM OVER 70 VOLUNTEER MEDICAL PROFESSIONALS RECRUITED FROM ST. LOUIS' MOST REPUTABLE MEDICAL PRACTICES AND ORGANIZATIONS. THE CLINIC ALSO PROVIDES CHRONIC DISEASE MANAGEMENT AND EDUCATION, LAB TESTING, PRESCRIPTION ASSISTANCE, WOMEN'S HEALTH CARE AND OTHER ANCILLARY SERVICES.
PATIENTS WHO HAVE COMPLEX CONDITIONS ARE REFERRED TO OUR GUIDES FOR UNDERSTANDING, INFORMATION AND ACCESS (GUIA) PROGRAM. THE WORD GUIA MEANS "GUIDE" IN SPANISH, AND IS AT THE CORE OF WHAT THE PROGRAM DOES BY CREATING PATHWAYS TO CARE FOR PATIENTS WITH COMPLEX CONDITIONS (GENERALLY DEFINED AS THOSE WHO NEED REFERRALS TO TWO OR MORE OUTSIDE SPECIALISTS, OR WHO HAVE A CHRONIC DISEASE LIKE HYPERTENSION AND DIABETES). GUIA WORKS WITH OUR NETWORK OF OVER 100 OUTSIDE PROVIDERS TO CONNECT THE PATIENT TO NEEDED CARE, AND A TEAM MEMBER ACCOMPANIES THE PATIENT TO APPOINTMENTS, ACTS AS A PATIENT ADVOCATE AND HELPS PATIENTS NAVIGATE THE BURDENSOME FINANCIAL ASSISTANCE/CHARITY CARE PROCESSES. THE PROGRAM ALSO PROVIDES HEALTH EDUCATION PHONE CALLS AND A HOME VISITS FOR PATIENTS MANAGING HYPERTENSION AND DIABETES.
