504HEALTHNET WILL PROVIDE SUPPORT TO AN INTEGRATED AND COORDINATED NETWORK OF HEALTHCARE PROVIDERS IN THE GREATER NEW ORLEANS REGION, WHICH ARE WORKING TOGETHER TO ACHIEVE THESE GOALS:1.) INCREASING ACCESS TO COMPREHENSIVE PRIMARY CARE AND BEHAVIORAL HEALTH SERVICES TO ALL PEOPLE IRRESPECTIVE OF THE ABILITY TO PAY,2.) IMPROVING THE QUALITY AND BENEFITS OF CARE,3.) INCREASING SUSTAINABILITY, EFFECTIVENESS AND EFFICIENCY AMONG THE MEMBER ORGANIZATIONS,4.) DEVELOPING LINKAGES FOR A FULL CONTINUUM OF HEALTH SERVICES,5.) PROVIDING ADVOCACY FOR THE NEEDS OF COMMUNITY-BASED PRIMARY CARE AND BEHAVIORAL HEALTH PROVIDERS SERVING UNDERSERVED POPULATIONS, AND6.) ADVANCING AN EFFECTIVE MEDICAL HOME MODEL SYSTEM OF CARE.

PUBLIC POLICY - 504HEALTHNET SUPPORTS POLICIES AND PROGRAMS THAT HELP IMPROVE ACCESS TO HEALTHCARE ACROSS LOUISIANA. THIS INCLUDES SUPPORT AND ADVOCACY ON BEHALF OF PROGRAMS SUCH AS THE GREATER NEW ORLEANS COMMUNITY HEALTH CONNECTION (GNOCHC), AND OPPORTUNITIES MADE AVAILABLE THROUGH THE AFFORDABLE CARE ACT. THESE ADVOCACY EFFORTS ENSURE SERVICES ARE AVAILABLE AND CARE IS PROVIDED THROUGH COMMUNITY HEALTH CENTERS WHICH ARE DELIVERING CARE TO THE MOST VULNERABLE POPULATIONS IN NEW ORLEANS.


OUTREACH AND EDUCATION:504HEALTHCONNECT- 504HEALTHNET IS ACTIVE IN HELPING RESIDENTS' ACCESS HIGH QUALITY HEALTH CARE IN THEIR COMMUNITY BY CONNECTING RESIDENTS, BOTH WITH AND WITHOUT HEALTH INSURANCE COVERAGE, WITH A PRIMARY CARE PHYSICIAN.


MEMBER SERVICES - 504HEALTHNET ORGANIZATION MEMBERS HAVE A COMMITMENT TO PROVIDING HIGH QUALITY MEDICAL CARE TO RESIDENTS OF THE GREATER NEW ORLEANS AREA, IRRESPECTIVE OF THEIR ABILITY TO PAY. MEMBERSHIP FEES ARE ON A SLIDING SCALE BASED ON THE SIZE OF THE ORGANIZATION, AND EVERY MEMBER HAS A SEAT ON THE BOARD.
