CONTINUING TO EMPOWER INTELLECTUALLY AND DEVELOPMENTALLY DISABLED ADULTS WITH OPPORTUNITIES FOR INDEPENDENCE BY PROVIDING EXCEPTIONAL COMMUNITY-BASED PERSON CENTERED PROGRAMS, THROUGH DEDICATED STAFF AND RESPONSIBLE MEASURABLE GOVERNANCE.

DAYBREAK PROVIDED HABILITATIVE SERVICES TO INDIVIDUALS WITH DEVELOPMENTAL DISABILITIES WHO ARE IN THE MEDICAID WAIVER PROGRAM. DURING FISCAL 2021, DAYBREAK PROVIDED 31,807 DAY HABILITATION SERVICES TO INDIVIDUALS WITH DEVELOPMENTAL DISABILITIES. A UNIT OF SERVICE IS MEASURED BY A SET NUMBER OF HOURS AN INDIVIDUAL SPENDS AT ONE OF THE ORGANIZATION'S DAY HABILITATION SITES WORKING ON DAILY ACTIVITIES AS WELL AS INTEGRATION IN THE COMMUNITY.
DAYBREAK PROVIDED HABILITATIVE SERVICES TO INDIVIDUALS WITH DEVELOPMENTAL DISABILITIES WHO LIVE IN THEIR OWN HOME OR FAMILY HOME. DURING FISCAL 2021, DAYBREAK PROVIDED 31,166 COMMUNITY HABILITATION SERVICES TO INDIVIDUALS WITH DEVELOPMENTAL DISABILITIES. A UNIT OF SERVICE IS MEASURED BY A SET AMOUNT OF TIME, AND THE INDIVIDUALS ARE APPROVED FOR SERVICES TO BE RENDERED IN THEIR OWN HOME OR IN THE COMMUNITY.
DAYBREAK PROVIDED INDIVIDUAL RESIDENTIAL ALTERNATIVE SERVICES TO INDIVIDUALS WITH INTELLECTUAL AND DEVELOPMENTAL DISABILTIES. DURING FISCAL 2021 DAYBREAK PROVIDED 2,366 UNITS OF SERVICE. SUPERVISED RESIDENCES PROVIDE SUPERVISION 24 HOURS A DAY, 7 DAYS A WEEK.
