CHCGD OPERATES SIX COMMUNITY HEALTH CENTERS LOCATED IN MONTGOMERY COUNTY, OHIO, FOR THE PURPOSE OF PROVIDING PRIMARY AND PREVENTIVE HEALTH CARE SERVICES TO PATIENTS IN MEDICALLY UNDER-SERVED AREAS, REGARDLESS OF THEIR ABILITY TO PAY. CHCGD ALSO OPERATES A MOBILE HEALTH UNIT FOR SCHOOL AGED YOUTHS IN CONJUNCTION WITH THE DAYTON CITY SCHOOLS. IN 2021, CHCGD PROVIDED A TOTAL OF 54,181 ENCOUNTERS TO 21,009 PATIENTS. CHCGD RECEIVED $749,612 WORTH OF DONATED PHARMACEUTICALS FROM OHIO MEDICAID TO PROVIDE VACCINATIONS TO ELIGIBLE CHILDREN UNDER ITS VACCINE FOR CHILDREN (VFC) PROGRAM.
