412 YOUTH ZONE - THE 412 YOUTH ZONE PROGRAM IS DESIGNED FOR ANY ALLEGHENY COUNTY YOUTH AGES 16-23 THAT ARE IN FOSTER CARE, WERE PREVIOUSLY IN FOSTER CARE OR ARE HOMELESS. PROGRAMMING IS COMPREHENSIVE AND PROVIDES CASE MANAGEMENT, LIFE SKILLS, AND BEHAVIORAL HEALTH, EDUCATION AND WORKFORCE DEVELOPMENT.


FOSTER CARE - THE PROGRAM IS DESIGNED TO PROVIDE A SAFE AND SECURE HOME ENVIRONMENT FOR THOSE CHILDREN AND YOUTH WHO CANNOT LIVE WITH THEIR BIRTH FAMILIES CURRENTLY. THE ULTIMATE GOAL IS FAMILY REUNIFICATION OR PERMANENCY PLANNING WITH AN ALTERNATIVE FAMILY SETTING OR ADOPTION. THE PROGRAM INCLUDES THE OPTION OF EMERGENCY FOSTER/SHELTER CARE BEDS, AS NEEDED. SERVICES ARE TARGETED TO YOUTH AGES 0-21 REFERRED BY COUNTY CHILD PROTECTION.


RAPID REHOUSING - PROVIDES CASE MANAGEMENT, RENTAL ASSISTANCE AND RESOURCE COORDINATION TO ENABLE YOUNG ADULTS TO GET OFF THE STREET AND INTO A HOME.
