TO PROVIDE BEHAVIORAL HEALTH SERVICES TO INDIVIDUALS AND FAMILIES WITH MENTAL HEALTH OR SUBSTANCE ABUSE AND RELATED PROBLEMS.

SOCIAL REHABILITATION - THE SITE BASED PSYCHIATRIC REHABILITATION PROGRAM IS DAY PROGRAM THAT OFFERS GROUPS TOUCHING IN THE DOMAINS OF LIVING, LEARNING, WORKING, SOCIAL AND SELF-MAINTENANCE. THE PROGRAM RUNS GROUPS DAILY FROM 9-4 AND THE GROUP SCHEDULES RUN 8 WEEKS. CONSUMERS ARE ABLE TO CHOOSE GROUPS THAT FIT THEIR SPECIFIC RECOVERY NEEDS. EACH CONSUMER IS ASSIGNED A PSYCH REHAB SPECIALIST THAT THEY MEET WITH ONE ON ONE BI-MONTHLY TO UPDATE RECOVERY PLANS AND PROVIDE CASE MANAGEMENT.


OUTPATIENT AND MEDICAL SERVICES - PROVIDED CARE TO 1,701 SERVICE PARTICIPANTS AT OUR FACILITIES. SERVICE PARTICIPANTS RANGED IN AGES FROM 3 TO 85 YEARS OLD. THESE SERVICES PROVIDE COUNSELING AND MEDICATION MANAGEMENT TO INDIVIDUALS WITH A MENTAL HEALTH DIAGNOSIS. 91% REPORT THAT TREATMENT RECEIVED HAS HAD A BETTER EFFECT ON THEIR QUALITY OF LIFE. THESE SERVICES ALLOW THE SERVICE PARTICIPANT TO CONTINUE MAINTAINING THEIR EVERYDAY LIFE ROUTINES IN THEIR COMMUNITY, WHILE IN THE LEAST RESTRICTIVE LEVEL OF CARE. GOALS INCLUDE ONGOING EDUCATION AND ADVOCACY TOWARD EMPOWERING SERVICE PARTICIPANTS IN THEIR RECOVERY. OTHER GOALS INCLUDE THE CONTINUATION OF THE PROCESS OF TRANSFORMATION TO A RECOVERY ORIENTED MENTAL HEALTH SERVICE SYSTEM.


RESIDENTIAL SERVICES/COMMUNITY INTEGRATION SERVICES PROVIDED CARE AND TREATMENT IN THE PAST YEAR TO 79 INDIVIDUALS, AGES 20-70. THE POPULATION SERVED ARE THOSE WITH SEVERE MENTAL ILLNESS. FROM JULY 1, 2021 TO JUNE 30, 2022, 78% OF THE 43 CONSUMERS DISCHARGED HAVE TRANSITIONED TO A LESS STRUCTURED SETTING. GOALS ARE TO IDENTIFY COMMUNITY RESOURCES THAT WILL ENHANCE THE SERVICE PARTICIPANTS PERSONAL RESOURCES. AND INTEGRATE CONSUMERS BACK INTO THE COMMUNITY AS QUICKLY AS POSSIBLE. GOALS ALSO INCLUDE ONGOING EDUCATION AND ADVOCACY TOWARD EMPOWERING SERVICE PARTICIPANTS IN THEIR RECOVERY.
