COMMUNITY OUTREACH -THROUGH ITS COMMUNITY OUTREACHPROGRAM, ECDA SEEKS TO PROMOTE PREVENTION, EARLY DIAGNOSIS AND RESPONSIBLE SELF-MANAGEMENT OF DIABETES. THESE SERVICES ARE OFFERED TO ALL FACETS OF THE COMMUNITY TO INCLUDE THE UNDERSERVED, HIGH-RISK POPULATIONS, HEALTH SEEKERS, PLACES OF EMPLOYMENT AND HEALTHCARE PROFESSIONALS. ECDA PARTNERED WITH VARIOUS SECTORS OF THE COMMUNITY TO PROVIDE EDUCATION AND SUPPORT SERVICES FOR10,420 INDIVIDUALS AT 54 EVENTS INCLUDING HEALTH SCREENINGS, DISCUSSION GROUPS, COMMUNITY TALKS, AND HEALTH FAIRS. A WIDE RANGE OF SERVICES WERE RENDERED AT THESE EVENTS TO INCLUDE INDIVIDUAL DIABETES RISKASSESSMENTS, SELF-MANAGEMENT EDUCATION, AND DISTRIBUTION OF CURRENT EDUCATIONAL LITERATURE. ADDITIONALLY, TWO ISSUES OF THE "IN TOUCH WITH DIABETES" EDUCATIONAL NEWSLETTER WERE PUBLISHED AND 10,000 COPIES WEREDISTRIBUTED THROUGHOUT THE COUNTY.--NURSE INFORMATION AND REFERRAL - THE RN RESPONDED TO 931 INDIVIDUAL DIABETES-RELATED ASSISTANCE REQUESTS. ECDA FOLLOWS THE STANDARDS OF THE AMERICAN DIABETES ASSOCIATION'S SELF-MANAGEMENT EDUCATIONAL GUIDELINESFOR THESE REQUESTS. THROUGH THIS PROGRAM, ECDA PROVIDES INFORMATION, REFERRAL AND EDUCATION RESOURCES TO HELP INDIVIDUALS EFFECTIVELY NAVIGATE THE HEALTHCARE SYSTEM AND SELF-MANAGE THEIR DIABETES.--EMERGENCY SUPPLY EXCHANGE - ECDA MADE 176 DISTRIBUTIONS OF DIABETES MEDICAL SUPPLIES INCLUDING GLUCOMETERS, METER STRIPS, LANCETS, SYRINGES AND PUMP SUPPLIES TO UNINSURED AND UNDERINSURED INDIVIDUALS TOTALING ANESTIMATED VALUE OF $40,000. THESE INDIVIDUALS ALSO RECEIVED FREE NURSE INSTRUCTION ON PROPER USAGE OF THE DISTRIBUTED SUPPLIES. THE NURSE ALSO PROVIDED CONSULTATION ON FORMULATING A PLAN FOR THESE INDIVIDUALS TO ACCESS THEIR SUPPORT ON A LONG-TERM BASIS.--YOUTH FITNESS PROGRAM - ECDA DELIVERED A RESEARCH-BASED PROGRAM CALLED HOPSPORTS TO 126 4TH, 5TH AND 6TH, GRADE STUDENTS WITH EITHER A BODY MASS INDEX OR ACTIVITY LEVELS OUTSIDE THE RECOMMENDED GUIDELINES. THE HOPSPORTS PROGRAM FEATURES STATE-OF-THE ART TECHNOLOGY TO ENGAGE YOUTHIN INTENSE PHYSICAL FITNESS ACTIVITIES AND NUTRITION EDUCATION IN AN EFFORT TO REDUCE THEIR RISK FOR TYPE 2 DIABETES AND OTHER LONG-TERM CONSEQUENCES OF CHILDHOOD OBESITY. THROUGHOUT THE YEAR, PARTICIPATING STUDENTS AVERAGED 160 MINUTES OF MODERATELY VIGOROUS PHYSICAL ACTIVITIES WHILE DEVELOPING THE FUNDAMENTAL SKILLS TO PERFORM SPORTS, MARTIAL ARTS, CORE DRILLS, DANCE,AND BASIC TUMBLING.
