NUTRITION PROGRAMS - PROVIDES MEALS TO THE ELDERLY IN THEIR HOMES AND AT VARIOUS CONGREGATE SITES.


HOME CARE - THE PURCHASED SERVICES COMPONENT PERMITS THE ORGANIZATION TO SUBCONTRACT WITH AREA PROVIDERS FOR VARIOUS HOME CARE SERVICES FOR THE ELDERLY. THE CASE MANAGEMENT COMPONENT PROVIDES FOR ADMINISTRATIVE SUPPORT OF THE HOME CARE PROGRAM.


ENHANCED SERVICES - PROVIDES SERVICES THAT TARGET FRAIL ELDERS FOR THE PURPOSE OF PREVENTING OR DELAYING NURSING HOME PLACEMENT.
