ACMH ALSO OPERATES, INDEPENDENT OF ITS HOUSING, A CARE MANAGEMENT PROGRAM PROVIDING OUTREACH, ENGAGEMENT AND CARE COORDINATION TO ADULTS WITH SERIOUS MENTAL ILLNESS AND/OR OTHER CHRONIC HEALTH CONDITIONS AND HOME AND COMMUNITY BASED SERVICES, WHICH INCLUDE HABILITATION, PSYCHOSOCIAL REHABILITATION, AND PEER AND FAMILY SUPPORTS, PROVIDED IN THE COMMUNITY TO ADULTS WITH BEHAVIORAL HEALTH DISORDERS WITH THE HIGHEST RISK OF HOSPITALIZATION OR INSTITUTIONALIZATION.
