CARE MANAGEMENT SERVICES - WE ACHIEVE OUTCOMES IN CRITICAL LIFE AREAS SUCH AS HOUSING, MAINTAINING WELLNESS, ACQUIRING BENEFITS, OBTAINING PAID WORK OR A VOLUNTEER POSITION, BUILDING SOCIAL SUPPORTS AND MAINTAINING PERSONAL FINANCES. OUR EMPHASIS IS TO FILL THE GAPS IN PUBLIC BENEFITS TO ASSURE HAPPINESS, HEALTH AND MEANING IN EVERYONE'S LIFE. SOMETIMES WE WORK WITH A PERSON OVER A SPECIFIC PERIOD OF TIME AND THEY END SERVICES MORE STABLE AND CONNECTED. OTHER PEOPLE HAVE LONGER ENGAGEMENTS THAT SPAN YEARS OR EVEN THEIR LIFETIME. WE ASSIST THE PERSON OR FAMILY WITH UNDERSTANDING HOW TO SET UP A SPECIAL NEEDS TRUST, DEEPENING THEIR UNDERSTANDING OF OPTIONS AND POSSIBILITIES.
