BERGEN'S PROMISE PROVIDES CARE MANAGEMENT SERVICES FOR FAMILIES WITH YOUTH AGES 5 TO 21 WHO ARE DEALING WITH BEHAVIORAL, MENTAL HEALTH, SUBSTANCE ABUSE AND INTELLECTUAL/DEVELOPMENTAL DISABILITY CHALLENGES. THE GOAL IS TO DEVELOP A COMMUNITY BASED SERVICE PLAN THAT IS FOCUSED ON ACHIEVING LONG-TERM SUCCESS.
