HOSPICE & PALLIATIVE CARE, INC. PROVIDES COMFORT AND SUPPORT FOR THOSE WITH LIFE-LIMITING ILLNESSES, AND FOR THEIR FAMILIES, BY ATTENDING TO THEIR PHYSICAL, EMOTIONAL, AND SPIRITUAL NEEDS. OUR TEAM OF PROFESSIONALS INCLUDES DOCTORS, NURSES, NURSES AIDES, SOCIAL WORKERS, CHAPLAINS, VOLUNTEERS AND GRIEF COUNSELORS WHO EMPOWER AND EDUCATE FAMILIES DURING THIS TIME. WE COORDINATE CARE BETWEEN TEAM MEMBERS AND FAMILY MEMBERS TO BEST MEET PATIENTS AND FAMILIES NEEDS. IN 2021, WE SERVED 935 UNDUPLICATED PATIENTS.
