-SAGE ELDERCARE IS A UNIQUE COMMUNITY RESOURCE THAT OFFERS A COMPREHENSIVE ARRAY OF PROGRAMS AND SERVICES FOR OLDER ADULTS AND CAREGIVERS, PROMOTING INDEPENDENCE AND QUALITY OF LIFE.

SAGE ELDERCARE'S HOMECARE: SAGE PROVIDES IN-HOME CARE FOR OLDER ADULTS AND MUCH NEEDED RESPITE, SUPPORT AND PEACE OF MIND FOR FAMILY CAREGIVERS. ACCREDITED BY THE COMMISSION ON ACCREDITATION FOR HOME CARE, SAGE'S EXPERIENCED PROFESSIONAL STAFF DELIVER QUALITY CARE THAT MEETS THE INDIVIDUAL NEEDS OF OLDER ADULTS AND THEIR FAMILIES. REGULAR VISITS WITH A CLIENT, THE FAMILY AND THE HOME HEALTH AIDES PROVIDES AN OPPORTUNITY TO ADDRESS CONCERNS AND MAKE APPRORIATE ADJUSTMENTS IN THE PLAN OF CARE. THE SAGE SOCIAL WORKER IS AVAILABLE TO FAMILIES WHO NEED ADDITIONAL SUPPORT OR COUNSELING. HOMECARE CAN BE PROVIDED ON AN HOURLY, OVERNIGHT OR LIVE-IN BASIS, AND MAY INCLUDE HELP WITH PERSONAL CARE, FAMILY RESPITE, AND HOMEMAKING. THESE SERVICES ARE PROVIDED BY NEW JERSEY BOARD OF NURSING CERTIFIED HOME HEALTH AIDES AND SUPERVISED BY REGISTERED NURSES.HOMECARE ALLEVIATES SOME OF THE OVERHWLEMING RESPONCIBILITIES OF FAMILIES CARING FOR AGING LOVED ONES. WITH THE PROGRAM'S IN-HOME ASSISTANCE, CAREGIVERS AND FAMILY MEMBERS RECEIVE MUCH-NEEDED RESPITE AND TIME TO DEVOTE TO COMPETING RESPONSIBILITIES. IN 2021, 55 CERTIFIED HOME HEALTH AIDES AND TWO REGISTERED NURSES PROVIDED OVER 37,606 HOURS OF PERSONAL CARE TO 66 OLDER ADULTS.


SAGE ELDERCARE'S SPEND-A-DAY ADULT DAY HEALTH CARE CENTER: SINCE 1975, SPEND-A-DAY HAS OFFERED FRAIL AND VULNERABLE OLDER ADULTS DAYS FULL OF MEANINGFUL ACTIVITIES THAT STIMULATE MIND AND BODY, HEALTH MONITORING BY REGISTERED NURSES, AND NUTRITIOUS SNACKS AND HOT NOON MEALS ALL PROVIDED WITHIN A SAFE, SECURE, HOME-AWAY-FROM-HOME ENVIRONMENT. PARTICIPANTS DISPLAY IMPROVEMENTS IN COGNITIVE AND PHYSICAL HEALTH AND MAINTAIN THEIR LEVEL OF INDEPENDENCE THROUGH ENGAGEMENT, PEER INTERACTION AND SUPPORT. PARTICIPATION IN THE SPEND-A-DAY PROGRAM PROVIDES FAMILY CAREGIVERS WITH MUCH NEEDED RESPITE FROM THE STRESSES AND STRAINS OF THEIR CAREGIVING ROLE AND ALLOWS THEM TO ATTEND TO OTHER RESPONSIBILITIES. LICENSED BY THE DEPARTMENT OF HEALTH AND SENIOR SERVICES SINCE 1999, SPEND-A-DAY OFFERS DAILY PROGRAMMING THAT ACCOMMODATES THE VARIOUS INTERESTS AND NEEDS OF ITS PARTICIPANTS WITH STAFF WHO ARE EXPERTS IN DEALING WITH A MULTITUDE OF CONDITIONS INCLUDING EARLY, MIDDLE AND LATE STAGES OF ALZHEIMER'S DISEASE AND RELATED DEMENTIAS, LIMITED MOBILITY, POST-STROKE, DIABETES, PARKINSON'S, POST-FRACTURE, CARDIAC ILLNESSES, AND DEPRESSION. A FULL-TIME REGISTERED NURSE PROVIDES DAILY NURSING CARE AND ASSISTANCE WITH MEDICATION MANAGEMENT AND A FULL-TIME LICENSED SOCIAL WORKER PROVIDES COUNSELING AND SUPPORT FOR PARTICIPANTS AND THEIR FAMILIES. THE CENTER ALSO OFFERS A FULL RANGE OF SUPPORTIVE HEALTH SERVICES SUCH AS PHYSICAL, SPEECH AND OCCUPATIONAL THERAPIES, NUTRITIONAL COUNSELING AND MORE. THE SPEND-A-DAY PROGRAM HAD TO CLOSE IN-PERSON PARTICIPATION IN MARCH OF 2020 AFTER THE STATE MANDATED DECISION TO SUSPEND ALL MEDICAL DAY CENTERS DURING THE HEIGHT OF THE COVID-19.IN JULY 2021, SPEND-A-DAY RE-OPENED AFTER EXTENSIVE CLEANING AND SANITATION OF THE FACILITY AND HVAC SYSTEM. GUIDANCE FROM THE CDC AND THE NJ DEPARTMENT OF HEALTH WAS FOLLOWED, AND LOCAL AUTHORITIES AND OTHER MEDICAL PROFESSIONALS ADVISED ON REOPENING PLANS. SPACE WAS RECONFIGURED, PPE AND OTHER SAFETY ESSENTIALS WERE PURCHASED IN ORDER TO PRACTICE SOCIAL DISTANCING AND ADDITIONAL PRECAUTIOUS WERE IMPLEMENTED. IN 2021, SPEND-A-DAY PROVIDED OVER 9,513 HOURS OF CARE AND TELEHEALTH DAYS TO 64 PARTICIPANTS PLUS RESPITE SUPPORT FOR THEIR CAREGIVERS.


SAGE ELDERCARE MEALS ON WHEELS AND GROCERY SHOPPING: SAGE ELDERCARE MEALS ON WHEELS: IN 2021, 52,200 PROFESSIONALLY PLANNED AND PREPARED MEALS WERE DELIVERED TO 173 OLDER ADULTS IN NORTHERN UNION COUNTY, MILLBURN-SHORT HILLS, AND CHATHAM WITH THE HELP OF VOLUNTEERS. SAGE'S MEALS ON WHEELS, ONE OF THE FIRST SUCH PROGRAMS IN NEW JERSEY, WAS ESTABLISHED IN 1966. MEALS ON WHEELS PROVIDES HOME DELIVERED MEALS EACH WEEKDAY TO INDIVIDUALS AGED 60 AND OLDER WHO ARE HOMEBOUND AND UNABLE TO COOK FOR THEMSELVES. THE MEALS ON WHEELS PROGRAM NOT ONLY HELPS WITH FOOD INSECURITY AMONG OLDER ADULTS, BUT IT ADDRESSES THE ISSUE OF SOCIAL ISOLATION BY PROVIDING VITAL WELLNESS CHECKS. MANY RECIPIENTS REPORT THAT THE MEALS ON WHEELS VOLUNTEERS ARE THE ONLY VISITS THEY RECEIVE DURING THE WEEK. SAGE'S SOCIAL WORKER AND OTHER SAGE PROFESSIONALS ASSIST WHEN STAFF AND VOLUNTEERS REPORT ANY CONCERNS ABOUT A PROGRAM RECIPIENT. THE DEPARTMENT MAINTAINS A FOOD PANTRY AND IS AVAILABLE WHEN ADDITIONAL FOOD IS NEEDED. THE PRECAUTIONS CREATED BY COVID-19 HAVE INCREASED THE DEMAND FOR SAGE'S MEALS ON WHEELS PROGRAM. NOW, MORE THAN EVER SAGE REMAINS A CRUCIAL RESOURCE FOR OUR REGIONS OLDER ADULT'S NUTRITIONAL AND SOCIAL NEEDS.GROCERY SHOPPING AND ERRAND SERVICE: SAGE GROCERY SHOPS FOR OLDER ADULTS FOR A NOMINAL FEE. STAFF MEMBERS MEET A GROUP OF VOLUNTEERS AT THE SUPERMARKET EACH WEEK. THE VOLUNTEERS SHOP AND DELIVER THE GROCERIES TO EACH PERSON'S HOME. IN 2021, 373 SHOPPING TRIPS WERE MADE FOR 19 OLDER ADULTS. THE VOLUNTEERS DELIVERING THE MEALS AND DOING THE GROCERY SHOPPING ALSO PROVIDE AN ESSENTIAL WELFARE VISIT FOR MANY OLDER ADULTS WHO MIGHT OTHERWISE GO WITHOUT SEEING ANYONE FOR DAYS.


SAGE ELDERCARE OTHER PROGRAMS: INFOCARE PROVIDES SOCIAL SERVICE SUPPORT, REFERRALS, RESOURCES AND CAREGIVER SUPPORT; COMMUNITY EDUCATION PROGRAMS OFFER A BROAD RANGE OF EDUCATIONAL PROGRAMS AND WORKSHOPS, EXERCISE CLASSES, AND SUPPORT GROUPS TO PROMOTE WELLNESS FOR OLDER ADULTS AND THEIR CAREGIVERS; AND S.H.I.P. (STATE HEALTH INSURANCE ASSISTANCE) PROGRAM PROVIDES INFORMATION ABOUT MEDICARE, GAP COVERAGE AND PRESCRIPTION DRUG PLANS. YOUR DECISIONS MATTER IS DESIGNED TO HELP FAMILY MEMBERS, FRIENDS AND HEALTHCARE PROFESSIONALS HAVE A CONVERSATION ABOUT END-OF-LIFE DECISION MAKING IDEALLY BEFORE A MEDICAL CRISIS OCCURS.
