THE VERMONT ASSOCIATION OF AREA AGENCIES ON AGING (V4A) SUPPORTS AND STRENGTHENS VERMONT'S FIVE AREA AGENCIES ON AGING (AAA'S) TO ENSURE THAT ALL VERMONTERS ARE ABLE TO AGE WITH HEALTH, INDEPENDANCE AND DIGNITY

Part 3 - Line 4a
PROVIDE ASSESSMENT AND CASE MANAGEMENT SERVICES UNDER THE DEPARTMENT OF VETERANS' AFFAIRS VETERAN-DIRECTED HOME AND COMMUNITY BASED SERVICES PROGRAM. SERVICES ARE PROVIDED THROUGH CONTRACTS WITH THE FIVE VERMONT AREA AGENCIES ON AGING AND OTHER ORGANIZATIONS.
PROVIDE SUPPORT TO THE FIVE VERMONT AREA AGENCIES ON AGING. SUPPORT INCLUDES COMPUTER TECH DEVELOPMENT, PUBLIC AWARENESS, AND ADMINISTRATION OF STATEWIDE CONTRACTS.
PROVIDED PROGRAMS OF TRAINING FOR CASE MANAGERS EMPLOYED BY LOCAL AREA AGENCIES ON AGING. HOME HEALTH AGENCIES AND OTHER SIMILAR ORGANIZATIONS.
