THIS WILL BE ACCOMPLISHED THROUGH THE IMMEDIATE PROVISION OF COORDINATED SERVICES; REUNIFICATION OF FAMILIES WHERE APPROPRIATE; THE PROVISION OF SUPERVISED EMERGENCY AND TRANSITIONAL HOUSING, WITH THE GOAL OF PERSONAL RESPONSIBILITY AND SELF-SUFFICIENCY.

OUR EMERGENCY SHELTER PROVIDES BED NIGHTS TO YOUNG ADULTS AGES 16-24 OUR TLP PROGRAM PROVIDES BED NIGHTS TO YOUNG ADULTS AGES 16-24 AND INFANTS/TODDLERS OUR HOMELESSNESS PREVENTION PROGRAM ASSISTS HOUSEHOLDS WITH FINANCIAL ASSISTANCE AND CASH MANAGEMENT SERVICES TO KEEP THEM FROM EVICTION. OUR RAPID RE-HOUSING PROGRAM ASSISTS HOMELESS HOUSEHOLDS TO FIND AND MAINTAIN PERMANENT HOUSING. OUR HEALTHY PARENTING AND MENTORING PROGRAM SERVES MANY LOCAL FAMILIES. OUR STREET OUTREACH PROGRAM PROVIDES SERVICES TO YOUNG ADULTS SUCH AS FOOD, CLOTHING, CASE MANAGEMENT SERVICES AND EDUCATIONAL AND MEDICAL SUPPORT.
