THROUGH OUR NURSING HOME TRANSITION PROGRAMS WE TRANSITIONED 12 INDIVIDUALS OUT OF NURSING HOMES INTO THE COMMUNITY USING THE OHS SUBSIDY. AS AN AGENCY BILS HELPED APPROXIMATELY 2,000 INDIVIDUALS DURING THE YEAR IN VARIOUS AREAS INCLUDING BENEFITS, HOUSING, INFORMATION AND REFERRAL, VOCATIONAL SERVICES, ETC..
