THE SENIOR CARE OPTIONS PROGRAM PROVIDES SERVICES TO SENIORS WHO ARE ELIGIBLE FOR MEDICAID AND MAY BE ELIGIBLE FOR MEDICARE. THESE CONSUMERS RECEIVED A HEALTH-CARE PLAN INDIVIDUALIZED FOR THEIR PERSONAL NEEDS.


THE COMMUNITY CHOICES PROGRAM PROVIDES HOME AND COMMUNITY-BASED SUPPORT SERVICES, CARE MANAGEMENT AND COORDINATION TO VERY FRAIL, LOW-INCOME ELDERS OVER SIXTY AND ON MEDICAID SO THEY MAY REMAIN LIVING AT HOME RATHER THAN IN INSTITUTIONS.


STATE HOME CARE PROGRAM PROVIDES HOME AND COMMUNITY-BASED SUPPORT SERVICES, CARE MANAGEMENT AND COORDINATION TO FRAIL, LOW-INCOME ELDERS OVER THE AGE OF SIXTY.
