THE HOME CARE PROGRAM PROVIDED HOME CARE SUPPORT FOR 2,939 FRAIL ELDERS AND CASE MANAGEMENT SERVICES TO 2,683 FRAIL ELDERS IN FISCAL YEAR 2021.
THE FALLON NAVICARE PROGRAM PROVIDED SENIOR CARE SERVICES TO 2,426 INCOME ELIGIBLE ELDERS IN FISCAL YEAR 2021.
DURING FISCAL YEAR 2021, THE TITLE III NUTRITION PROGRAM PROVIDED 461,712 MEALS TO 3,691 ELDERS.
