INDIVIDUAL ADVOCACY PROVIDE CASE MANAGEMENT, HEALTH INSURANCE COUNSELING, INFORMATION AND REFERRAL, AS WELL AS CONTRACTS FOR LEGAL CONSULTATION AND MENTAL HEALTH SERVICES FOR OLDER VERMONTERS


INDEPENDENT LIVING PROVIDE FUNDING FOR AND OVERSIGHT TO PROVIDERS OF HOME-DELIVERED MEALS, CONGREGATE MEALS AND TRANSPORTATION SERVICES TO 2,000 ELDERS


CAREGIVER SUPPORT PROVIDE ONGOING SUPPORT FOR CAREGIVERS THRU RESPITE AND EDUCATIONAL PROGRAMS AIMED AT THE UNDERSTANDING THAT WITHOUT SUPPORT TO CAREGIVERS THOSE INFLICTED WITH COGNITIVE IMPAIRMENTS ARE OFTEN PLACED IN INSTITUTIONS
