SCLS IS DESIGNATED BY THE NYS DEPT OF EDUCATION TO PROVIDE SERVICES AND PASS THRU GRANT AND AID TO MEMBER LIBRARIES LOCATED IN SUFFOLK COUNTY.

MISSION:

The goal of this organization is to work to relieve poverty in developing countries by providing funds, technical assistance and supplies which improve the health, education, welfare, social well-being and self-reliance of individuals and families. Also included are organizations that provide relief services in response to a major disaster or large-scale emergency that occurs abroad.

NSOLF IS DEDICATED TO PRESERVING THE CHARACTER AND NATURAL BEAUTY OF THE TOWN OF NORTH SALEM, NY AND THE AREA AROUND IT BY ESTABLISHING LAND AREAS FREE FROM EXPLOITATION, DEVELOPMENT, AND DETERIORATION AND BY MAINTAINING THE ENVIRONMENTAL AND ECOLOGICAL INTEGRITY OF THE AREA.

TO ASSIST THE FACULTY OF THE DEPARTMENT OF PEDIATRICS AT THE UNIVERSITY AT BUFFALO IN THE DELIVERY OF HEALTH CARE. The goal of this organization is to build, rehabilitate, manage and/or provide rental housing for low-income individuals and families, older adults and people with disabilities; or which make purchasable housing available to low or moderate income families by offering lower priced housing and/or affordable payment plans, by arranging for interest or mortgage subsidies or by involving eventual owners in the construction process (sweat equity).

THE BROCKPORT AUXILIARY SERVICE CORPORATION IS AN INNOVATIVE AND ENTREPRENURIAL ORGANIZATION THAT DELIVERS KEY SERVICES WITH INTEGRITY AND EFFICIENCY TO FOSTER STUDENT SUCCESS, ENHANCE THE QUALITY OF LIFE, AND SUPPORT THE STRATEGIC INITIATIVES AT SUNY BROCKPORT.

MISSION:

MAIN ACTIVITIES ARE SUPPORT THE SHEN YUN SHOW PRODUCTION, SHOW PROMOTION AND SHOW PERFORMANCES. RAISING AWARENESS ABOUT TRADITIONAL CHINESE CULTURE, ESPECIALLY ABOUT SHEN YUN PERFORMING ARTS AND FEI TIAN COLLEGE.

The goal of this organization are the private foundations that make grants based on charitable endowments. Because of their endowments, they are focused primarily on grantmaking and generally do not actively raise funds or seek public financial support. These are the most common type of private foundation. They are generally endowed, usually from a single individual or family. Private foundations are considered family foundations if relatives or the original donor are still active on the board of trustees or in the operation of the foundation.

MISSION:

The goal of this organization is to administer centralized fundraising efforts and then in turn distribute the contributed funds to several nonprofit agencies. United Way and community chests or funds, the United Jewish Appeal and other religious appeals are examples of federated giving programs. (rev. 1/05)

FROM A HUMBLE BEGINNING IN BAY SHORE IN 1985 AS A PROVIDER OF HOUSING FOR ADULTS LIVING WITH MENTAL ILLNESS, OUR HOUSING PROGRAMS NOW ALSO INCLUDE THE ELDERLY, LIMITED INCOME FAMILIES WITH CHILDREN AS WELL AS PERSONS WITH SPECIAL NEEDS AND VETERANS. MERCY HAVEN HAS GROWN THROUGHOUT ITS 38 YEARS TO BECOME A MULTIFACETED COMMUNITY RESOURCE. WE HAVE BROUGHT SERVICES TO BOTH NASSAU AND SUFFOLK COUNTIES IN NEW YORK AND ARE DEDICATED MEMBERS OF THE COMMUNITIES WE SERVE. IN ADDITION TO THE HOUSING PROGRAMS DESCRIBED IN PART III OF THIS FORM, MERCY HAVEN OPERATES THE FOLLOWING PROGRAMS: PLEASANT GARDENS ADULT HOME PROVIDES A HOME AND PERSONAL CARE FOR 26 OLDER ADULTS WHO ARE UNABLE TO LIVE INDEPENDENTLY. PLEASANT GARDENS IS LICENSED BY NEW YORK STATE DEPARTMENT OF HEALTH AND PROVIDES RESIDENTS WITH SERVICES THAT INCLUDE COOKING, CLEANING, LAUNDRY SERVICE, AND MEDICATION MANAGEMENT THAT ARE PROVIDED BY STAFF WHO ARE ON-SITE 24 HOURS PER DAY, 7 DAYS PER WEEK. VISITS FROM A MEDICAL DOCTOR AND OTH
