THESE ORGANIZATIONS ARE DEEPLY ROOTED WITHIN THE COMMUNITIES THEY SERVE. CCF IS DESIGNATED BY THE NYS DEPARTMENT OF HEALTH AS A LEAD HEALTH HOME SERVING CHILDREN AND A DESIGNATED HOME AND COMMUNITY-BASED SERVICES (HCBS) PROVIDER. OUR MISSION IS TO KEEP CHILDREN AND FAMILIES IN NEW YORK HEALTHY AND ON THEIR DEVELOPMENTAL TRAJECTORY. CCF PROVIDES TRAINING, OVERSIGHT, AUDITING, COMPLIANCE, PARTNERSHIP NETWORKING, ADVOCACY, AND BACK-OFFICE SUPPORT SO THAT OUR MEMBER AGENCIES CAN FOCUS ON THEIR CORE MISSIONS OF SERVING AND HELPING NEW YORK'S CHILDREN AND FAMILIES TO LEAD HEALTHY AND HAPPY LIVES.
