BON SECOURS CHARITY HEALTH SYSTEM, INC. (BSCHS) IS THE PARENT AND CENTRAL ORGANIZATION OF AN ESTABLISHED INTEGRATED HEALTHCARE DELIVERY SYSTEM. BSCHS, THROUGH ITS SUBSIDIARIES AND AFFILIATES, PROVIDES A BROAD RANGE OF HEALTHCARE AND RELATED SERVICES TO COMMUNITIES IN THE SEVEN-COUNTY, TRI-STATE AREA THAT INCLUDES ROCKLAND, ORANGE, AND SULLIVAN COUNTIES IN NEW YORK, BERGEN, PASSAIC, AND SUSSEX COUNTIES IN NORTHERN NEW JERSEY, AND PIKE COUNTY IN PENNSYLVANIA.


BON SECOURS CHARITY HEALTH SYSTEM, INC. IS THE SOLE MEMBER OF HEALTHY COMMUNITIES ACO, LLC, WHICH WAS FORMED TO PARTICIPATE IN PROGRAMS AND TO RECEIVE AND DISTRIBUTE SHARED SAVINGS AND OTHER PAYMENTS RECEIVED FROM THIRD PARTY PAYORS, INCLUDING MEDICARE, UNDER QUALITY IMPROVEMENT ARRANGEMENTS AND TO REPAY SHARED LOSSES, IF ANY, OR OTHER MONIES DETERMINED TO BE OWED PURSUANT TO SUCH ARRANGEMENTS. THERE WERE NO DISTRIBUTIONS IN 2021.
