SHELTER PROGRAMWITH NEARLY 35 YEARS OF EXPERIENCE WORKING WITH FAMILIES IN NEED, THE ORGANIZATION HAS DEVELOPED A SET OF SERVICES THAT EMPOWER FAMILIES TO MOVE FROM HOMELESSNESS TO INDEPENDENT, SELF-SUFFICIENT LIVES IN THE COMMUNITY. THE ORGANIZATION'S SAN JOSE FAMILY SHELTER IS THE ONLY SHELTER IN SANTA CLARA COUNTY THAT EXCLUSIVELY SERVES HOMELESS FAMILIES. IT OPERATES 35 EFFICIENCY APARTMENTS 365 DAYS PER YEAR; DESPITE THE CHALLENGES AND UNCERTAINTY, THIS YEAR THE ORGANIZATION HAS BEEN ABLE TO SERVE 482 INDIVIDUALS, 287 OF THEM CHILDREN, WITH 52,000 SHELTER NIGHTS, AND186,000 NUTRITIOUS MEALS ALONG WITH SUPPORTIVE SERVICES AND EDUCATIONAL, BEHAVIORAL, CAREER, AND THERAPEUTIC SUPPORT. EIGHTY THREE PERCENT OF OUR SHELTER FAMILIES SUCCESSFULLY TRANSITIONED INTO PERMANENT HOUSING AND SELF-SUFFICIENCY. THE ORGANIZATION CONTINUES TO IDENTIFY CREATIVE SOLUTIONS AND ENGAGE WITH ITS PARTNER AGENCIES TO DEVELOP OPPORTUNITIES TO HELP FAMILIES IN THEIR CARE.HOMEWORK ENRICHMENT PROGRAM: PROVIDES DAILY, SUPERVISED ACADEMIC ASSISTANCE,SKILL DEVELOPMENT AND ENRICHMENT ACTIVITIES TO ABOUT 150 CHILDREN IN GRADES K-12 EACH YEAR. DURING THE YEAR, THE ORGANIZATION CREATIVELY PROVIDED OVER 850 HOURS OF SUPPORT DURING THIS UNPRECEDENTED TIME.VOYAGER DAYCARE: PROVIDES RELIABLE, QUALITY, ONSITE CHILDCARE FOR INFANTS AND PRESCHOOL CHILDREN AT OUR SAN JOSE FAMILY SHELTER. VOYAGER IS STATE LICENSED AND FREE OF CHARGE TO FAMILIES STAYING AT THE SHELTER. BY PROVIDING A SAFE AND LOVING ENVIRONMENT FOR THEIR YOUNG CHILDREN, THE ORGANIZATION ALLOWS PARENTS TO CONCENTRATE THEIR TIME AND ENERGY ON SEEKING EMPLOYMENT AND PERMANENT HOUSING.RAPID REHOUSING: IN COLLABORATION WITH THE CITY OF SAN JOSE, THE RAPID REHOUSING PROGRAM PROVIDES SHORT-TERM SUBSIDIES TO ATTAIN AND RETAIN STABLE HOUSING. FINANCIAL ASSISTANCE IS TIED TO CASE MANAGEMENT EFFORTS THAT FOCUS ON GROWING INCOME, ATTAINING LIVING-WAGE CAREERS, AND TRANSITIONING IN PLACE IN EXISTING HOUSING LEASES. ALL FAMILIES THAT PARTICIPATE IN THE SAN JOSE FAMILY SHELTER PROGRAM ARE ASSESSED FOR ELIGIBILITY TO THIS SUBSIDY PROGRAM, WHICH BENEFITS FAMILIES WITH MODERATE HOUSING BARRIERS.HOMELESS PREVENTION SYSTEM (HPS)/ASSISTED RAPID RESOLUTION: THIS PROGRAM FOCUSES ON ASSESSING AND FINANCIALLY ASSISTING FAMILIES AT THE BRINK OF HOMELESSNESS FOR RESOLUTION AND AVOIDING THE HOMELESS SYSTEM. THE ORGANIZATION IS RESPONSIBLE FOR BOTH IN-HOUSE CASE MANAGEMENT AND DIRECT FINANCIAL ASSISTANCE AS WELL AS REFERRAL TO OTHER PARTNERS IN THE HPS SYSTEM.

HOMELESS PREVENTION SYSTEM/COVID-19 FINANCIAL ASSISTANCE PROGRAMIN DIRECT RESPONSE TO THE FINANCIAL CRISIS CREATED BY COVID-19-RELATED INCOME DISRUPTION, SACRED HEART COMMUNITY SERVICE AND DESTINATION: HOME CREATED A PROGRAM DEDICATED TO PROVIDING FINANCIAL ASSISTANCE TO THOSE IN NEED IN SANTA CLARA COUNTY. JOINING IN PARTNERSHIP WITH OTHER COMMUNITY ORGANIZATIONS, THE ORGANIZATION HAS HELPED, AND CONTINUES TO HELP, THOUSANDS OF FAMILIES WHO HAVE BEEN FINANCIALLY AFFECTED BY COVID-19. FOR THE YEAR ENDED SEPTEMBER 30, 2021, THE ORGANIZATION HAS SERVED OVER 300 FAMILIES AVOID EVICTION. ALL THE WHILE, FAMILY SUPPORTIVE HOUSING, INC.
