MEDICAL CASE MANAGEMENT PROGRAM - OVER THE COURSE OF THE FISCAL YEAR COVERED BY THIS 990, WESTERN NORTH CAROLINA AIDS PROJECT (WNCAP) PROVIDED SERVICES TO 380 PERSONS LIVING WITH HIV TRANSPORTATION TO ACCESS OTHER AREA RESOURCES; HIV TREATMENT ADHERENCE MONITORING AND COUNSELING; FACILITATION OF DIALOGUE BETWEEN CLIENT, AND THEIR CARE TEAM; ASSISTANCE TO HELP CLIENTS INTERPRET THEIR LAB RESULTS AND CARE; FOOD PANTRY ASSISTANCE; SUPPORT AND ADVOCACY IN OFFICIAL SETTINGS, SUCH AS COURT PROCEEDINGS; ANNUAL ASSESSMENTS OF NEED; AND REFERRALS TO LOCAL RESOURCES. OR BEHAVIORAL HEALTH CARE, EMERGENCY FINANCIAL ASSISTANCE TO PREVENT HOMELESSNESS, AND A PEER NAVIGATOR PROGRAM.

PREVENTION PROGRAM - OVER THE COURSE OF THE FISCAL YEAR COVERED BY THIS 990, WNCAP PROVIDED THE FOLLOWING SERVICES TO PREVENT THE TRANSMISSION OF HIV AND OTHER SEXUALLY TRANSMITTED INFECTIONS (STI): CONDUCTED OVER 100 STI PREVENTION EDUCATION SESSIONS IN COMMUNITY SETTINGS, WITH 1,215 TOTAL PARTICIPANTS; DISTRIBUTED OVER 95,000 FREE CONDOMS TO ALMOST 50 DISTRIBUTION SITES IN THE COMMUNITY; ADMINISTERED 605 HIV TESTS, 529 HEPATITIS C TESTS, AND 94 TESTS FOR OTHER STIS INCLUDING CHLAMYDIA, GONORRHEA, AND SYPHILIS; DISTRIBUTED OVER 14,000 OVERDOSE REVERSAL KITS; PROVIDED NAVIGATION SERVICES TO OVER 180 INDIVIDUALS; AND REFERRED OVER 150 INDIVIDUALS TO HEALTH CARE (MEDICAL AND/OR MENTAL).
HOUSING PROGRAM - OVER THE COURSE OF THE FISCAL YEAR COVERED BY THIS 990, WNCAP PROVIDED THE FOLLOWING SERVICES TO PREVENT HOMELESSNESS AND/OR PROMOTE HOUSING STABILITY FOR PEOPLE LIVING WITH HIV (PLWH): PERMANENT HOUSING VOUCHERS TO OVER 50 PLWH; EMERGENCY FINANCIAL ASSISTANCE TO COVER HOUSING COSTS TO OVER 80 PLWH; EMERGENCY TEMPORARY HOUSING TO MEDICALLY FRAGILE INDIVIDUALS DURING THE ONSET OF THE COVID PANDEMIC; AND HOUSED 9 NEW INDIVIDUALS WHO WERE PREVIOUSLY HOMELESS.
