DURING THE FISCAL YEAR COVERED BY THIS FILING AND AS PART OF BETH ISRAEL LAHEY HEALTH AT HOME, CAREGROUP PARMENTER HOME CARE & HOSPICE CARED FOR PATIENTS THROUGH ITS AFFILIATE, NORTHEAST PROFESSIONAL REGISTRY OF NURSES D/B/A BETH ISRAEL LAHEY HEALTH AT HOME (BILH AT HOME). BILH AT HOME PROVIDED CARE TO PATIENTS BY PARTNERING WITH PHYSICIANS, HOSPITALS AND SKILLED FACILITIES, TO HELP PATIENTS MAKE A SEAMLESS TRANSITION BACK HOME IN THE EVENT OF A HOSPITALIZATION. BILH AT HOME ALSO OFFERS A RANGE OF PROGRAMS TO MEET THE COMPLEX NEEDS OF PATIENTS AND THEIR FAMILIES COPING WITH ADVANCED ILLNESS.
