MISSOURI MEDICAID EQUITY PROJECT. COMMUNITY HEALTH OUTREACH AND EDUCATION TO INCREASE ENROLLMENT OF NEWLY ELIGIBLE ADULTS IN EXPANDED MEDICAID AND IMPROVE INDIVIDUAL ACCESS TO CARE THROUGHOUT THE STATE OF MISSOURI.


PROVIDE QUALITY AFFORDABLE HOUSING, FOOD AND PERSONAL PROTECTIVE EQUIPMENT TO LOW INCOME INDIVIDUALS LIVING WITH CHRONIC DISEASES AND OTHER LONG TERM DISABILITIES IN ORDER TO IMPROVE POSITIVE LONG TERM HEALTH OUTCOMES.


PROVIDING EDUCATION AND INFORMATION ABOUT PREVENTION, TESTING AND TREATMENT OF STD, HIV, AND NCV. PROMOTION OF ABSTINENCE AND RISK REDUCTION METHODS FOR PREVENTING THE SPREAD OF STD'S, HCV AND HIV. PROMOTING EARLY DETECTION OF INFECTIOUS DISEASES IN ORDER TO FACILITATE MEDICAL CARE AND TREATMENT AND PREVENT ONGOING HIGH RISK BEHAVIOR.
