DIRECT PATIENT CARE - THE ORGANIZATION'S TEAM OF SPECIALISTS TAILORS A PLAN OF CARE FOR EACH PATIENT AND FAMILY FACING A TERMINAL ILLNESS. THE PLAN CONSIDERS PHYSICAL, EMOTIONAL, SOCIAL, AND SPIRITUAL NEEDS. THE TEAM INCLUDES THE PHYSICIAN, NURSE, SOCIAL WORKER, CERTIFIED NURSING ASSISTANT, VOLUNTEERS, AND SPIRITUAL CARE STAFF. 1125 PATIENTS SERVED


PALLIATIVE CARE - PALLIATIVE CARE IS PROFESSIONAL, COMPASSIONATE, HOLISTIC CARE FOR INDIVIDUALS WITH AN ADVANCED ILLNESS, CHRONIC CONDITION, OR A LIFE-LIMITING DISEASE. IT FOCUSES ON HELPING PATIENTS AND THEIR FAMILIES IMPROVE QUALITY OF LIFE BY ALLEVIATING SYMPTOMS AND PAIN. THE ORGANIZATION OFFERS PALLIATIVE CARE CONSULTATION AND SERVICES ON REFERRAL AND PARTNERS WITH MERCY HEALTH PARTNERS TO OFFER PALLIATIVE CARE SERVICES IN AN INPATIENT SETTING. 904 PATIENTS SERVED


BEREAVEMENT CARE - GRIEF SUPPORT SERVICES ARE DELIVERED BY LICENSED PROFESSIONALS WHO WORK WITH FAMILIES FOR UP TO 13 MONTHS AFTER THE DEATH OF THEIR LOVED ONE. GRIEF SUPPORT INCLUDES MONTHLY MAILINGS, SUPPORT GROUPS, CAMP COURAGE FOR CHILDREN, SCHOOL PROGRAMS, AND INDIVIDUAL COUNSELING SESSIONS. ALL SERVICES ARE FREE AND OPEN TO ANYONE IN THE COMMUNITY. 269 CLIENTS SERVED


COMMUNITY SERVICE AND OTHER MISCELLANEOUS PROGRAMS - THE ORGANIZATION PROVIDES END OF LIFE EDUCATION TO THE COMMUNITY BY PARTICIPATING IN REGIONAL COMMITTEES AND CONFERENCES, HEALTH FAIRS, PUBLIC SPEAKING, AND PRESENTATIONS. AS A COMMUNITY-BASED ORGANIZATION, IT IS THE ORGANIZATION'S MISSION TO INCREASE ACCESS TO HOSPICE CARE BY EDUCATING THE COMMUNITY, PARTICULARLY TERMINALLY ILL PATIENTS AND THEIR FAMILY MEMBERS, ABOUT THE BENEFITS OF EARLY REFERRAL TO HOSPICE. INFORMATION ABOUT THE SERVICES OF THE ORGANIZATION SUCH AS PAIN CONTROL, SYMPTOM MANAGEMENT, IN-HOME ASSISTANCE AND GRIEF AND LOSS SUPPORT IS PROVIDED.
