CASE MANAGEMENT ENSURES THAT INDIVIDUALS WHO COME TO US IN NEED ARE ASSESSED FOR ALL THEIR NEEDS, NOT JUST THEIR PRESENTING PROBLEM. CASE MANAGEMENT SERVICES PROVIDE INTERNAL AND EXTERNAL REFERRALS FOR THE SERVICES TO LIFT INDIVIDUALS AND FAMILIES OUT OF THEIR SITUATION, REMOVE BARRIERS TO LIVING IN A SAFE ENVIRONMENT, AND MAXIMIZE SELF-SUFFICIENCY AND WELL-BEING. TOTAL CLIENTS SERVED: 94,518


NUTRITION SERVICES ASSISTS LOW-INCOME INDIVIDUALS AND FAMILIES WITH MEETING A VERY BASIC NEED. PREPARED MEALS ARE AVAILABLE AT OUR SUPPER PROGRAMS ACROSS COOK AND LAKE COUNTIES, AS WELL AS HOME DELIVERED MEALS FOR SENIORS AND NUTRITIOUS MEALS FOR PRE-SCHOOL AND SCHOOL AGE CHILDREN. ONE TRANSITIONAL SHELTER PROVIDES IMMEDIATE RELIEF TO PEOPLE FACING IMMINENT HOMELESSNESS. OTHER HOUSING PROGRAMS ASSIST INDIVIDUALS WITH LOCATING, OBTAINING, AND FURNISHING APARTMENTS IN COMMUNITIES ACROSS THE SERVICE AREA. ALSO PROVIDES SOME HOUSING SERVICES IN THE FORM OF RENTAL ASSISTANCE TO KEEP PEOPLE WHO ARE IN DANGER OF EVICTION FOR FINANCIAL REASONS IN THEIR APARTMENTS. TOTAL CLIENTS SERVED: 4,036
