THE ORGANIZATION'S MISSION IS TO PROVIDE COMPASSIONATE HOSPICE CARE IN AN INNOVATIVE AND PERSONALIZED MANNER TO THOSE IN NEED, WITHIN THE RESOURCES AVAILABLE, ENABLING THEM TO PRESERVE THEIR DIGNITY AND MAINTAIN THEIR HIGHEST LEVEL OF INDEPENDENCE. THE ORGANIZATION SERVES PATIENTS IN PORTER COUNTY AS WELL AS PORTIONS OF LAKE, JASPER, STARKE, AND LAPORTE COUNTIES.

THE VNA HOSPICE PROGRAM BRINGS COMFORT TO PERSONS LIVING WITH A TERMINAL ILLNESS AND PROVIDES SUPPORT TO THEM AS WELL AS TO THEIR FAMILIES. PATIENTS ARE ABLE TO LIVE THEIR FINAL DAYS WITH PEACE AND DIGNITY THROUGH EFFECTIVE PAIN MANAGEMENT AND SYMPTOM CONTROL IN ADDITION TO PSYCHOSOCIAL, EMOTIONAL, AND SPIRITUAL SUPPORT. VNA'S HOSPICE CARE IS PROVIDED BY AN INTERDISCIPLINARY TEAM OF CAREGIVERS, INCLUDING THE PATIENT'S PHYSICIAN, REGISTERED NURSES, SOCIAL WORKERS, VOLUNTEERS, CLERGY, OCCUPATIONAL AND PHYSICAL THERAPISTS, CHAPLAINS, AIDES AND DIETICIANS IN THE PATIENT'S HOME, NURSING HOME, ASSISTED LIVING FACILITY OR AT THE VNA ARTHUR B. AND ETHEL V. HORTON HOSPICE CENTER. FOR THE YEAR ENDED SEPTEMBER 30, 2021, HOSPICE SERVICES WERE PROVIDED TO 884 PATIENTS COMPRISING OVER 35,000 DAYS OF SERVICE.


THE VNA OF NORTHWEST INDIANA PROVIDES SERVICES TO ASSIST THE ILL, ELDERLY AND DISABLED IN REMAINING SAFE AND INDEPENDENT AT HOME. THIS ASSISTIVE CARE INCLUDES MEALS ON WHEELS FOR ELDERLY AND DISABLED RESIDENTS WHO ARE UNABLE TO PREPARE NUTRITIOUS MEALS FOR THEMSELVES AND THE LIFELINE PERSONAL EMERGENCY RESPONSE SYSTEM. VNA ALSO OFFERS A MEDICAL ALERT SYSTEM WHICH NOTIFIES THE CLIENTS CAREGIVERS OF ANY FALLS. THESE SERVICES ENABLE PATIENTS TO REMAIN IN THEIR HOMES IN A SAFE ENVIRONMENT. VNA ALSO HAS A BEREAVEMENT GREIF AND HEALING CENTER FOR CHILDREN AND YOUNG ADULTS IN THE COMMUNITY. FOR THIS YEAR ENDING SEPTEMBER 30, 2021, 626 INDIVIDUALS RECEIVED THESE SERVICES COMBINED.


THE VNA OF NORTHWEST INDIANA PROVIDES PALLIATIVE CARE SERVICES TO INDVIDUALS WHO HAVE A TERMINAL ILLNESS BUT WHO STILL WISH TO SEEK CURATIVE MEASURES. THESE ARE PATIENTS THAT MIGHT OTHERWISE ELECT HOSPICE CARE, BUT STILL ELECT TO PURSUE CURATIVE CARE MEASURES. THEY REQUIRE COMFORT CARE, CARE COORDINATION AND CONSULTATION TO ASSURE THAT THEY CAN REMAIN AS INDEPENDENT AND IN CHARGE OF THEIR CARE CHOICES FOR AS LONG AS POSSIBLE. FOR THE YEAR ENDING SEPTEMBER 30, 2021, 317 INDIVIDUALS RECEIVED THESE SERVICES.
