PERMANENT SUPPORTIVE HOUSING: COMMUNITY HOUSING NETWORK (CHN) IS A NONPROFIT HOUSING COMPANY THAT DEVELOPS, OWNS AND MANAGES PERMANENT SUPPORTIVE HOUSING DEDICATED TO VULNERABLE POPULATIONS INCLUDING PEOPLE WITH MENTAL HEALTH CONDITIONS, SUBSTANCE USE DISORDERS, AND HISTORIES OF HOMELESSNESS. SINCE OUR BEGINNING IN 1987, CHN'S PERMANENTSUPPORTIVE HOUSING PROGRAM HAS PROVEN THAT VULNERABLE POPULATIONS CAN LIVE SUCCESSFULLY IN THE COMMUNITY WHEN THEIR HOUSING IS CONNECTED TO APPROPRIATE SUPPORTIVE SERVICES. CHN'S HOUSING ALLOWS RESIDENTS TO LIVE STABLY, WHICH IN TURN HAS POSITIVE EFFECTS ON THEIR HEALTH,PRODUCTIVITY AND CONNECTION TO THE COMMUNITY. IN ADDITION, ONCE STABLY HOUSED, CHN'S RESIDENTS USE FEWER COSTLY COMMUNITY RESOURCES LIKE EMERGENCY HEALTH CARE AND HOMELESS SHELTERS, GENERATING SIGNIFICANT COST SAVINGS TO PUBLIC SYSTEMS. TODAY, CHN PROVIDES OVER 1,900PERMANENT SUPPORTIVE HOUSING UNITS AND IS AN INTEGRAL PART OUR COMMUNITY'S EFFORT TO IMPROVE THE LIVES OF OUR MOST VULNERABLE CITIZENS.CHN PROVIDES ALL RESIDENTS WITH A RENT SUBSIDY TO ENSURE OUR HOUSING IS AFFORDABLE, AS WELL AS SAFE, DEPENDABLE, AND WELL MAINTAINED. CHN ALSO OFFERS A CONTINUUM OF SUPPORTIVE HOUSING SERVICE MODELS, INCLUDING MODELS WITH 24/7 ON-SITE WELL-BEING MONITORING, ON-SITE SERVICES,RESIDENT MANAGERS AND CONNECTIONS TO COMMUNITY-BASED SOCIAL, HEALTH AND EMPLOYMENT SERVICES. IN ADDITION TO ITS OWN SERVICES, CHN ENGAGES WITH OTHER SERVICE AGENCIES TO OFFER RESIDENTS MORE ACCESS TO BEHAVIORAL HEALTH, WELLNESS AND EMPLOYMENT SERVICES. SUCCESS IS MEASURED BY LENGTH OF STAY IN OUR HOUSING, SUCCESSFUL EXITS, INCLUDING EXITS TO MORE INDEPENDENT HOUSING, BEING CONNECTED TO AS-NEEDED SUPPORTIVE SERVICES, AND INCREASED INCOME, INCLUDING THROUGH EMPLOYMENT. BY INCLUDING A SAFETY NET OF SERVICES IN ALL ITS HOUSING, CHN'S HOUSING EQUIPS RESIDENTS WITH THE TOOLS THEY NEED TO OVERCOME OBSTACLES IN THEIR LIVES AND MAINTAIN STABILITY. STABILITY ALLOWS RESIDENTS TO GAIN INDEPENDENCE AND BEGIN TO FLOURISH, INCREASING THEIR INCOMES AND FINDING MEANINGFUL EMPLOYMENT, BUILDING SUSTAINING RELATIONSHIPS, AND RECONNECTING TO THEIR LOVED ONES AND COMMUNITIES.


DEVELOPMENT: CHN HAS DEVELOPED, OWNS AND MANAGES MORE THAN 1,200 PERMANENT SUPPORTIVE HOUSING APARTMENTS LOCATED AT OVER 140 DIFFERENT SITES SCATTERED THROUGHOUT FRANKLIN COUNTY IN 32 ZIP CODES, INCLUDING SIX SUBURBAN COMMUNITIES. AS A REAL ESTATE DEVELOPER, CHN ACQUIRESPROPERTY, OBTAINS ZONING APPROVALS, ASSEMBLES FINANCING AND OVERSEES PROJECTS THROUGH CONCEPT, DESIGN, ENGINEERING, BIDDING AND CONSTRUCTION PHASES. DEVELOPMENTS INCLUDE BOTH NEW CONSTRUCTION AND ACQUISITION AND RENOVATION OF EXISTING BUILDINGS. CHN USES MANY FUNDING SOURCES TO DEVELOP ITS HOUSING, INCLUDING LOW-INCOME HOUSING TAX CREDITS, TAX-EXEMPT BONDS, STATE AND LOCAL FUNDING, AND PHILANTHROPIC GRANTS.CHN ALSO USES THE DEVELOPMENT PROCESS TO FORM LONG-TERM NEIGHBORHOOD CONNECTIONS THROUGH ITS COMMUNITY ACCEPTANCE APPROACH. CHN PROACTIVELY FORMS COMMUNITY ADVISORY COUNCILS PRIOR DEVELOPING PROPERTY TO INFORM NEIGHBORS ABOUT CHN AND ITS HOUSING MODELS. CHN ALSO SOLICITS NEIGHBORHOOD INPUT INTO THE HOUSING'S DESIGN AND OPERATIONS. BY OFFERING A FORUM FOR ON-GOING COMMUNICATION, THESE COUNCILS PROVIDE THE LONG-TERM RELATIONSHIPS THAT CONNECT CHN AND OUR FUTURE RESIDENTS TO OUR NEIGHBORHOODS.


COMMUNITY HOUSING NETWORK PROVIDES ALL CUSTOMARY PROPERTY MANAGEMENT FUNCTIONS FOR ITS PORTFOLIO OF SCATTERED-SITE APARTMENTS INCLUDING: 24-HOUR ON-CALL MAINTENANCE, TENANT SCREENING AND SELECTION, LEASE-UP, LEASE ENFORCEMENT, RENT COLLECTION, MAINTENANCE AND REPAIR, BUDGETING, PURCHASING AND ACCOUNTING.
