COMMUNITY-BASED ORGANIZATION PROVIDING A CONTINUUM OF COMPREHENSIVE CARE INCLUDING SKILLED HOME HEALTH, HOSPICE SERVICES, AND PERSONAL AIDE PRIVATE DUTY CARE TO RESIDENTS OF YORK AND ADAMS COUNTIES. SERVED 1,045 PATIENTS AND MADE 22,691 HOME VISITS. CARE PROVIDED TO INDIVIDUALS OF ALL AGES THROUGH ALL STAGES OF LIFE AND HAVE AN 87% PATIENT SATISFACTION FOR HOME HEALTH AND 93% PATIENT SATISFACTION FOR HOSPICE. THE PATIENT SATISFACTION RATINGS COVER THE PERIOD JANUARY 1, 2021 THROUGH DECEMBER 31, 2021. THE VNA OF HANOVER & SPRING GROVE RECEIVED A 2.5 STAR RATING FOR THE CENTERS FOR MEDICARE AND MEDICAID SERVICES PATIENT SATISFACTION RATING SYSTEM.
