HOUSING SERVICES: CASE MANAGEMENT; HOUSING SEARCH; PERMANENT SUPPORTIVE HOUSING; HOMELESS PREVENTION AND RE-HOUSING; RENTAL ASSISTANCE SUBSIDIES. DIRECTLY SUB-LEASED 52 SUBSIDIZED SCATTERED-SITE APARTMENTS TO FORMERLY HOMELESS AND DISABLED INDIVIDUALS AND FAMILIES. PROVIDED FURNITURE AND FURNISHINGS, UTILITY ASSISTANCE, AND PROVISION OF HOME-BASES STABILIZATION SERVICES TO A TOTAL OF 103 HOUSEHOLDS CONSISTING OF 147 PEOPLE. PROVIDED CASE MANAGEMENT AND HOUSING SEARCH BENEFITING 32 PEOPLE. PREVENTED 222 EVICTIONS HELPING 563 PEOPLE (INCLUDING CHILDREN) AVOID HOMELESSNESS.


ADULT SHELTER: OPERATED 16 BED ADULT EMERGENCY HOMELESS SHELTER; PROVIDED OVER 5,500 SHELTER BED NIGHTS; SHELTERED 30 INDIVIDUALS; 13 INDIVIDUALS MOVED INTO PERMANENT HOUSING.


FAMILY SHELTER: OPERATED 5 UNIT FAMILY EMERGENCY HOMELESS SHELTER; PROVIDED APPROXIMATELY 5,000 SHELTER BED NIGHTS; SHELTERED 13 FAMILIES CONSISTING OF 18 CHILDREN; 7 FAMILY HOUSEHOLDS MOVED INTO PERMANENT HOUSING.
