TO ASSIST PERSONS WITH DISABILITIES AND SENIORS THROUGH MANAGED LONG TERM SUPPORTS AND SERVICES TO OBTAIN HOME MODIFICATIONS THROUGH THEIR ASSIGNED MANAGED CARE ORGANIZATION BY ASSISTING THE MCO WITH COORDINATION, MANAGEMENT AND LOGISTICAL PROVISIONS FOR THE COMPLETION OF THE HOME MODIFICATION PROJECT.
