MISSION:

THE ORGANIZATION'S PRIMARY EXEMPT PURPOSE IS TO RAISE AND RECEIVE FUNDS IN FURTHERANCE OF THE MISSION AND PURPOSES OF AFFILIATE 501(C)(3) ORGANIZATIONS AND TO PROVIDE SUPPORT FOR COMMUNITY 501(C)(3) ORGANIZATIONS THROUGH ANGEL TEAR MINISTRIES.

MISSION:

CONCORDIA LUTHERAN HEALTH AND HUMAN CARE PROVIDES FAITH BASED, HIGH QUALITY SKILLED NURSING, SHORT TERM REHAB, PERSONAL CARE, ADULT DAY CARE SERVICES, RETIREMENT LIVING, AND CHILD CARE SERVICES. WE SERVE THOSE WITH LIMITED FUNDS TO THE BEST OF OUR ABILITY.

MISSION:

TO PROVIDE EXECUTIVE LEVEL OVERSIGHT, MANAGEMENT AND STRATEGIC PLANNING FOR THE NON-PROFIT HOME AND COMMUNITY BASED ENTITIES THAT DELIVER QUALITY HOME HEALTH SERVICES, HOSPICE SERVICES, AND SUPPLY NEEDS IN GOOD FAITH, ON BEHALF OF THOSE WHO ENTRUST THEMSELVES TO OUR CARE AND TO MINISTER TO THE UNIQUE NEEDS OF EACH INDIVIDUAL WITH RESPECT, DIGNITY AND COMPASSION.

MISSION:

HEALTH CARE DELIVERY AND MANAGEMENT: PROVIDE COMMUNITY BASED PROGRAMS, SERVICES, AND FACILITIES TO SENIORS AND DISABLED AND ILL PERSONS IN THEIR HOMES. INCLUDES NURSING, THERAPY, HOME HEALTH AIDE, MEDICAL SOCIAL WORK, AND RELATED SERVICES.

HARMONY PROTECTION GROUP'S CHARITABLE PURPOSE IS TO PROVIDE INSURANCE COVERAGE TO AFFILIATE 501(C)(3) ORGANIZATIONS.

MISSION:

HOME HOSPICE/NURSING: ESTABLISH AND MAINTAIN PROGRAMS WHICH PROVIDE SUPPORTIVE CARE FOR TERMINALLY ILL PATIENTS AND THEIR FAMILIES; AND TO PROVIDE ASSOCIATED SERVICES INCLUDING, BUT NOT LIMITED TO, OPERATION OF A NON-PROFIT HOSPICE.

TO SERVE OUR AGING COMMUNITY WITH A CONTINUUM OF HIGH-QUALITY CAREGIVING OPTIONS, PROVIDED IN A CHRISTIAN ENVIRONMENT, AND TO SERVE THOSE WITH LIMITED FUNDS TO THE BEST OF OUR ABILITY.

THE ORGANIZATION'S CHARITABLE PURPOSE IS TO SUPPORT AND CARRY OUT THE PURPOSES OF CONCORDIA LUTHERAN HEALTH AND HUMAN CARE (EIN 25-0969458) BY PROVIDING MEDICAL SERVICES TO ITS RESIDENTS AS WELL AS RESIDENTS OF THE COMMUNITY IN CABOT, PA REGARDLESS OF THEIR ABILITY TO PAY.
