TLC HEALTH NETWORK (TLC) IS A HEALTH SYSTEM AND DESIGNATED SAFETY NET/VITAL ACCESS PROVIDER IN WESTERN NEW YORK THAT OFFERS A VARIETY OF CRITICAL HEALTH SERVICES TO UNDERSERVED, RURAL COMMUNITIES OF CHAUTAUQUA, CATTARAUGUS, AND SOUTHERN ERIE COUNTIES. TLC WAS FORMED AS A RESULT OF A MERGER BETWEEN THE FORMER TRI-COUNTY MEMORIAL HOSPITAL IN GOWANDA, NY AND LAKE SHORE HOSPITAL (NOW LAKE SHORE HEALTH CARE CENTER) IN IRVING, NY. TLC OPERATES FACILITIES IN CHAUTAUQUA, CATTARAUGUS, AND ERIE COUNTIES INCLUDING LAKE SHORE HEALTH CARE CENTER, GOWANDA URGENT CARE & MEDICAL CENTER, FORESTVILLE PRIMARY CARE CENTER, AND OUTPATIENT CHEMICAL DEPENDENCY CLINICS IN CASSADAGA AND DERBY. WE ALSO OPERATE A HOME HEALTH CARE AGENCY THAT OPERATES THROUGHOUT THE TRI-COUNTY AREA.
